General Purpose: Provide general health education services to STRIDE patients with particular emphasis on diabetes and related chronic conditions. Essential Duties/Position Responsibilities: • Provide diabetes self-management education (DSMES) to STRIDE patients in one-on-one counseling sessions., • Utilize motivational interviewing to coach patients and reinforce self-management of SMART goals., • Under the purview of medical providers, provide diabetes medication education (including insulin), and diabetes medication titration (including insulin) according to pattern management principles for patients with Type 1 and Type 2 diabetes., • Under the purview of medical providers, initiate diabetes technologies (CGMs and insulin pumps), and follow up as needed. NOTE: do not need to be certified pump trainer., • Collaborate with providers and review patient data from the electronic health record (EHR) as needed to provide patient care., • Utilize interpretation services to communicate with non-English speaking patients., • Responsible for data tracking related to all health education services provided., • Participate in training staff as needed about diabetes medications and technologies., • Community: All are welcome at STRIDE – our patients are the center of our work., • Trust: Our patients feel safe, empowered, and supported., • Value: We respond to patients’ needs by providing effective, affordable care., • Required: Registered Dietitian (RD) or Registered Nurse (RN), BSN, • Active Certified Diabetes Care and Education Specialist (CDCES) or Board Certified- Advanced Diabetes Management (BC-ADM) certification preferred; can be actively working towards CDCES certification and must be obtained within 12 months of hire., • Fluent in English; Bi-lingual in Spanish highly preferred, • Two to four years’ experience providing diabetes education in individual or group settings preferred, • Experience with diverse communities preferred, • Working knowledge of diabetes self-management education services (DSMES)., • The ability to communicate effectively with individuals (patients and professionals) from a wide range of backgrounds, with particular emphasis on Hispanic/Latino cultures., • The ability to build effective, working relationships with patients and staff at different STRIDE clinic sites., • The ability to use a laptop computer and a variety of computer software applications; and strong interpersonal, problem solving, and leadership skills., • Must be able to work independently with minimal supervision., • Computer, and Electronic Medical Records (Epic), • Data and data-based development and resources, • Telephone, • Office equipment, • Interpretation line (Cyracom) Working Environment and Physical Activities: Office environment within a clinical setting. Enters data into computer programs via computer, mouse and keyboard. Moves about the office environment and occasionally to other locations. Moves/transports objects up to 25 lbs occasionally. Communicates information to others. Discerns/analyzes information from others to assist in decision making. Work Schedule Monday-Friday, 8 hours/day We offer a competitive salary/hourly range of $70,000 - $80,000, depending on your experience. This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training. Why STRIDE?
