Job Description Position Summary: We are seeking a highly skilled and compassionate Nurse Practitioner with experience in rheumatology and/or infusion therapy to join our small, multidisciplinary practice of 8 dedicated staff members. The ideal candidate will provide comprehensive patient care, manage chronic autoimmune and inflammatory conditions, and support our growing infusion program. This role offers flexibility for part-time or full-time schedules and the opportunity to work in a patient-centered, collaborative, and family-oriented environment where every team member plays an essential role in delivering personalized care. Key Responsibilities: • Evaluate, diagnose, and manage patients with rheumatologic conditions under physician supervision., • Conduct detailed patient assessments, order and interpret diagnostic tests, and develop individualized treatment plans., • Administer or oversee infusion therapies, including biologics and other rheumatology-related medications., • Monitor patient response to therapy, manage adverse effects, and adjust treatment plans as appropriate., • Educate patients and families about disease management, medications, and infusion procedures., • Collaborate closely with the physician, infusion nurse, and support staff to ensure seamless patient care., • Maintain accurate and thorough documentation in the Medent electronic medical record system., • Participate in quality improvement initiatives and ensure compliance with all clinical, OSHA, and HIPAA standards., • Support patient scheduling, follow-ups, and care coordination as needed., • Current and unrestricted Nurse Practitioner license in New York State., • Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)., • Minimum 2 years of experience in rheumatology, infusion therapy, or related specialty preferred., • Proficient in the administration and monitoring of IV infusions, including biologic therapies., • Strong clinical judgment and ability to manage complex patients with chronic autoimmune conditions., • Excellent communication, documentation, and interpersonal skills., • Ability to work independently and collaboratively in a small, fast-paced, and team-oriented environment., • Commitment to ongoing learning and maintaining professional certifications., • Openness to innovation and adapting to changes in healthcare practices and technologies. Compensation & Benefits: • Competitive salary: $98,000 – $120,000 annually, based on experience and infusion expertise., • Schedule: Monday–Thursday, 8:00 AM – 3:00 PM; Friday, 8:00 AM – 2:00 PM., • Continuing education and professional development support., • Collaborative, family-friendly, and flexible work environment., • Paid holidays and personal time after the probationary period., • Opportunity to help shape and grow the practice’s infusion and rheumatology services.
