Job Description The Surgical Nurse Navigator at ENT will guide patients and families through the healthcare system. The Nurse Navigator will serve as a single point of contact for patients and families by coordinating care and helping to ensure that their needs are met in a timely and efficient manner. The chosen candidate will work closely with other healthcare professionals to provide personalized care that is tailored to the individual patient’s needs. This position requires excellent communication, problem-solving, and analytic skills. As a member of the multidisciplinary team, the nurse navigator will serve as an advocate and educator for cancer patients, from diagnosis through survivorship. Their primary function is to ensure the patient remains compliant to the treatment plan by building relationships with patients and physicians, coordinating the plan of care, assisting with appointments, transportation needs, education, resource provisions and/or representation within the multidisciplinary care environment. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for cancer patients, including support with interdisciplinary and cross facility coverage and collaboration. Serves as a liaison between the patients and family, all physicians involved in the patient’s care, internal and external healthcare providers, support network members, and the wider healthcare community.
