Will be required to participate in the On-call Pharmacist rotation. Primary Responsibilities: • Reviews medication orders and evaluates appropriateness and completeness of orders with consideration given to the special needs and behaviors of the specific age group served. Obtains patient prescription and assists physician, as necessary, in formulating prescriptions and monitoring regimen. Enters medication orders into the computer system and dispenses medications in an accurate and timely manner in appropriate quantities based on stability, compatibility of the drug and age specific needs of the patient. Verifies all prescriptions not directly received from the physician or the physician's agent, • Assesses the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy, • Communicates with physicians in suggesting alternate or modified drug therapy programs that are the most cost effective. Communicates with the patient, physician, nursing staff, pharmacists and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure clinical outcomes is met, • Coordinates with nursing staff to assure the most cost-effective method of delivery is utilized to achieve expected outcome, • Information is input into the computerized software system to ensure accurate and timely dispensing and billing, • Coordinates the drug preparation work of pharmacy technician and checks the accuracy of that work prior to releasing the medications for distribution. Accurately checks medications prepared by pharmacy technicians, • Where applicable, compounds pharmaceuticals in an aseptic fashion according to literature guidelines. Compounds specialty drug products. Uses aseptic technique when compounding sterile drug products. Completes accurate documentation of items compounded and repackaged. Is familiar with, and strives to meet, various guidelines and recommendations as put forth by various governing bodies, • Be knowledgeable with regards to the proper use and handling of sterile and hazardous medications. Compounds and handles sterile and hazardous medications in accordance with established guidelines and regulations (i.e. USP <797>), • Ensures compliance with all FDA, DEA, State Board of Pharmacy, OSHA and other regulatory standards. Follows all department policies and procedures. Participates in department staff meetings and other functions required in the department, maintains license, continuing education and competencies as required by the appropriate regulatory agency and department, • Complies with all applicable Federal, State pharmacy rules and regulations related to dispensing, controlled substances, etc., • Ensures that all manufacturer requirements for REMS and/or other requirements are met when dispensing and/or shipping drugs, • Provide oversight of the safety; appropriateness, and effectiveness of the pharmaceutical care provide to patients (consumers) as it relates to specialty products dispensed You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Job DescriptionSUMMARY OF POSITION Dispenses medications, maintains drug inventory and patient and pharmacy records in a manner compliant with all federal, state, and local laws, regulations, and organizational policies and procedures. Ensures prescriptions are dispensed in compliance with 340B guidelines. Reasonable accommodations may be made to help enable qualified individuals with disabilities to perform the essential functions. • Read and interpret provider's medication orders, • Fill, process and dispense medication orders in an accurate manner, • Counsel patients with regards to medication use, side effects, interactions, and other pertinent information, • Ensure pharmacy compliance with regulatory bodies, • Ensure accuracy of adjudication of prescription claims, • Adhere to safety practices to ensure quality care for patients, • Collaborate with in-house providers regarding therapy equivalents, cost-savings measures and treatment plans, • Maintain pharmacy inventory through inventory control practices, • Provide customer service in answering phone calls and assisting patients in person in a prompt and courteous manner, • Administer therapeutic injections, • Triage and prioritize requests, • Compounds non-sterile preparations, • Reinforces instructions provided by the Pharmacist-in-Charge and Director of Pharmacy Maintains professional standards and certification in pharmaceutical practice including Kansas licensure and up-to-date continuing education. Accepts and uses supervision, direction and consultation to maximize available resources in the completion of work duties and to develop high-performing clinical support teams. Additional responsibilities will be assigned as needed to maintain and improve effective functioning of the department and to advance the mission of the organization. All job responsibilities will be carried out under the direction of the organization’s policies and procedures. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES • Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree required, • Kansas Board of Pharmacy Pharmacist Registration, • Certified Immunizer, • 2 years of community pharmacy practice experience Individual must: • Maintain continuing education per Kansas Board of Pharmacy requirements, • Possess strong prioritization skills and an ability to make decisions and delegate independently and effectively, • Be able to operate equipment such as computers, scan guns, tablet counters, and cash registers, • Maintain familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices, • Perform work accurately and thoroughly, • Communicate effectively verbally and in writing, • Maintain patient, team member and employer confidentiality and comply with all HIPAA regulations, • Be friendly, cheerful and helpful to patients and others and able to meet patients’ needs while following organizational policies and procedures, • Possess ability to pay attention to the minute details of a project or task, • Be able to adapt easily to changing conditions and work responsibilities, • Display a positive attitude and is a positive agent for change, • Work as part of a team and collaborate with co-workers, • Be able to complete assigned tasks under stressful situations, • Not be on the Office of Inspector General Exclusion List PREFERRED KNOWLEDGE, SKILLS, AND ABILITIES • Experience with 340B compliance, • Experience with Electronic Health Records preferred, • Experience in a FQHC or clinic setting, • Bilingual fluency in written and oral English/Spanish SUPERVISORY RESPONSIBILITY Pharmacy Technicians and Cashiers AMERICANS WITH DISABILITY SPECIFICATIONS Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
