Job Description SUMMARY OF POSITION Working as a member of pediatric care teams, the Healthy Development Program Coordinator works directly with patient families of infants and toddlers from birth through 5 years of age to provide developmental assessments, parent education and interventions. This role is to help support families by fostering healthy childhood development and managing and/or preventing developmental behavior issues commonly seen; referring families to the Care Coordinator or Community Health Worker teams to assist with internal and external community referrals. ESSENTIAL JOB RESULTS • Meeting with families at every well-child visit, either with the provider, or separately., • Assessing the child’s developmental progress in accordance with national standard guidelines as outlined by “CDC’s Developmental Milestones” and/or American Academy of Pediatrics “Bright Futures Milestones and Anticipatory Guidance” developmental surveillance., • Providing education and resources to parents to support families in fostering healthy childhood development, as well as, supporting the management of and providing anticipatory guidance for common developmental behavior issues such as, but not limited to, healthy sleep hygiene, colic, feeding concerns, toilet training, car seat safety, positive discipline, gross and fine motor development, cognitive development, social-emotional development, language development, oral health, safety and preventive anticipatory guidance in accordance with the child’s developmental age., • Identifying quality resources and initiating referrals to the Care Coordinator and Community Health Worker teams and following up with those team members as appropriate., • Assisting families in the scheduling of well-child visits so as to develop continuity of care for the family with their assigned provider care team., • Providing training sessions when appropriate to pediatric residents and students on a variety of topics in early childhood development if requested., • Entering results of developmental screening assessments in the electronic health record (EHR) system., • Attending required staff meetings and in-service trainings., • Participating in meetings with collaborative agency partners, referral agencies, as required, such as WIC, Infants and Toddler Services (ITS), to ensure timely knowledge of referral resources and that families are successfully accessing referred services., • Working with the clinic’s care coordination team to communicate family referrals so that results of referrals can be tracked and logged into the patient’s EHR., • Carrying out Vibrant Health’s mission: To provide access to respectful, empowering, culturally sensitive and appropriate high quality healthcare for the community, regardless of socio-economic obstacles., • Accepting and using supervision, direction and consultation to maximize available resources in the completion of work duties., • Additional responsibilities may be assigned as needed to maintain and improve effective functioning of the department and to advance the mission of the organization., • All job responsibilities will be carried out under the direction of the organization’s policies and procedures. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES • Experience and knowledge about early childhood growth and development, parent-child relationships, child health and family systems., • Experience assessing the growth and development of infants and children less than six years of age., • Demonstrated flexibility in working with an interdisciplinary team; ability to work in various roles and capacities, and to respect the boundaries of each role., • Experienced and comfortable working in a multi-cultural environment. Individual must: • Develop rapport with patients and their families, community and other health care personnel., • Exhibit a professional and respectful demeanor., • Work successfully independently and within a team, shifting tasks, prioritize tasks, problem-solve daily, and complete work within deadline., • Demonstrate excellent customer service, helpfulness, and a positive attitude., • Possess superior organizational skills with attention to detail and the ability to multi-task and meet deadlines., • Demonstrate the ability to recognize, understand, and appreciate the value of cultural diversity., • Demonstrate social and cultural sensitivity to ethnically and economically diverse patients and staff., • Exhibit experience and working knowledge of local community resources., • Hold current drivers’ license with an acceptable driving record, and reliable transportation., • Travel to all parts of the facility and throughout the community., • Will be required to have current CPR certification, which can be provided through Vibrant Health if needed., • Proficiency in use of personal computers and Microsoft Office (Outlook, Word and Excel). PREFERRED KNOWLEDGE, SKILLS, AND ABILITIES • Bilingual in English and Spanish., • Experience with a Federally Qualified Health Center or Safety Net Clinic., • Experience working with parents and/or facilitating parent groups., • Experience working in a medical setting or with health professional teams.
