Our mission is to deliver expert, compassionate wound care to patients across outpatient, home-based, and transitional settings. As we build from the ground up, we’re seeking a highly experienced, passionate Nurse Practitioner (NP) or Physician Assistant (PA) to play a foundational role in the development, success, and leadership of our clinical services. Key Responsibilities • Deliver comprehensive wound care services to patients across various care settings (homes, clinics, transitional care), • Perform advanced wound care techniques, including sharp debridement, biopsies, negative pressure wound therapy, and application of skin substitutes and grafts, • Evaluate and manage complex chronic wounds including pressure injuries, diabetic foot ulcers, venous ulcers, surgical wounds, and more, • Collaborate with referring providers, home health agencies, and SNFs to coordinate care and improve outcomes, • Serve as a clinical leader, helping to build and optimize protocols, documentation, and best practices, • Assist in the development of referral pathways and contribute to business growth, • Maintain detailed and timely documentation in EMR and ensure compliance with CMS and payer guidelines, • Participate in quality improvement initiatives and case review, • Educate patients and families on wound care, prevention, and follow-up, • Represent the company with professionalism, empathy, and integrity Qualifications • Active NP or PA license in the state of Arizona, • Minimum 2-3 years of advanced wound care experience required, • Proficient in sharp debridement, skin substitutes, grafts, and advanced modalities, • Strong understanding of CMS guidelines and wound documentation requirements, • Excellent clinical judgment, problem-solving, and time management skills, • Clear and professional communication and interpersonal skills, • Ability to work independently and manage a mobile caseload, • Passion for patient-centered care and practice growth, • Valid driver's license and willingness to travel primarily in the East Valley (mileage reimbursed) Benefits • Competitive salary based on experience, • Bonus potential after 12 months of employment, • Health insurance, • PTO, • Mileage and gas reimbursement for work-related travel, • Company vehicle provided after 12 months (based on performance and company growth), • Supportive, collaborative environment with leadership opportunities
