Job Description Registered Nurse – Hospice Case Manager (Home Health) After Hours Location: Toledo, Ohio Salary: Up to $85,000 annually Schedule: Full-Time | Flexible Hours | On-Call Rotation Guaranteed 40 hours per week even if fewer are worked Or client is seeking an experienced Registered Nurse (RN) Hospice Case Manager to support patients in their homes and provide high-quality, compassionate end-of-life care. This position includes after-hours triage calls, in-home symptom management visits, and death visits as needed. Schedule Details: • 4–5 days per week, 5pm–8pm, • Every other weekend required, • Perform patient assessments and develop individualized hospice care plans, • Provide symptom management, medication oversight, and comfort-focused interventions, • Respond to urgent needs, including death visits and immediate care situations, • Collaborate with families and educate them on end-of-life care needs, • Maintain accurate clinical documentation and ensure compliance with state and federal regulations Requirements: • Active RN license in the state of Ohio, • Minimum 1 year acute care or med-surg experience (Home Health or Hospice preferred), • Valid driver’s license and reliable transportation To Apply: Email resume to: Benefits: • Health insurance Work Location: In person
