About This Role As our LVN Home Care Supervisor, you’ll be the steady, supportive leader behind exceptional care in the home. You’ll guide and coach our in-home care team to ensure every participant receives care that is safe, reliable, and deeply respectful—always aligned with PACE standards and personalized care plans. This is a hands-on, field-forward role where you’ll own the daily rhythm of home care; scheduling, coverage, and service continuity, while providing real-time coaching through ride-a longs, check-ins, and competency support. You’ll also bring LVN-level clinical oversight where it matters most: completing in-home assessments within scope (like vitals and symptom checks), spotting early changes in condition, and escalating concerns quickly to the RN/Provider/IDT. You’ll work closely with the Interdisciplinary Team (IDT) to coordinate care, support smooth transitions across hospital/SNF/home, and ensure documentation and compliance stay strong—so participants and caregivers experience care that feels organized, consistent, and truly supportive.What does success look like as our Home Care Supervisor LVN?Lead with heart. Supervise, coach, and support Home Care staff (PCAs/Home Care Aides) to deliver safe, respectful, high-quality in-home care aligned with PACE standards, participant rights, and individualized care plans—providing real-time field support that reinforces dignity, safety, and service excellence.Keep care moving—every day. Manage daily operations including workload assignments, scheduling/coverage, attendance and call-outs, and timely completion of authorized services across participants’ homes to ensure continuity of care and minimize disruptions.Bring clinical oversight into the home. Provide LVN-level clinical oversight for home care services, including in-home assessments within scope (vitals, symptom checks, basic clinical observations, functional/safety snapshots), documentation of findings, and timely escalation of changes in condition or safety concerns to RN/Provider/IDT per protocol.Coach in real time. Conduct regular check-ins, field observations/ride-alongs, and competency validations; provide clear, consistent feedback and coaching, and reinforce infection prevention, safe body mechanics, and appropriate PPE use.Build and grow a strong team. Support hiring, onboarding, training, and ongoing development; lead performance management activities including recognition, corrective action, and development planning to promote engagement and retention.Partner with the IDT to protect participants. Collaborate closely with the IDT to implement care plans and support transitions (hospital/SNF/home); participate in IDT meetings as needed, provide updates on service delivery and observed needs/risks, ensure staff follow participant-specific instructions (ADL/IADL, transfers/mobility, safety precautions), and escalate concerns promptly (falls, functional decline, unsafe living conditions, suspected abuse/neglect, caregiver strain) per policy.Champion compliance and documentation. Ensure services comply with DHCS/CMS PACE requirements and internal policies, including confidentiality/HIPAA; oversee visit documentation and timekeeping (EVV/visit verification, service notes, incident reporting) for accuracy and timeliness.Own issues through resolution. Investigate, document, and resolve incidents, service failures, or complaints; implement corrective actions, track follow-through, and maintain positive relationships with participants and caregivers to support a consistently high-quality experience.Step in when it counts. During staffing shortages (PTO/call-outs/urgent gaps), provide direct participant care and/or complete critical visits within LVN scope to prevent missed services and maintain participant safety.Use data to improve the system. Identify trends and operational risks (repeat call-outs, missed visits, overtime drivers, participant dissatisfaction); track and report key metrics (visit completion, timeliness, feedback, incidents, productivity) and partner with Operations/Clinical leaders on continuous improvement, workflow standardization, and program growth.Model professionalism in every home. Maintain professionalism, cultural humility, and respectful communication in all participant homes and team interactions; perform other duties as assigned to support participant care, program operations, and regulatory compliance.Stay flexible to meet the moment. Take on additional duties as needed to support participant care, strengthen daily operations, and ensure ongoing regulatory compliance.What does an ideal candidate look like?Education that supports strong leadership. High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare, human services, business, or a related field preferred.An active LVN license in good standing. Active, unrestricted LVN license in the applicable state (CA, if applicable) required; maintains licensure without restrictions.Hands-on home care experience. 2+ years of experience in home care, community-based care, personal care services, or a similar setting required; prior lead/supervisor experience strongly preferred. Experience supporting older adults and/or medically complex populations required; PACE or other value-based/interdisciplinary care model experience preferred.Strong LVN clinical judgment (within scope). Demonstrated ability to support basic assessments (vitals, symptom observation), recognize changes in condition, escalate appropriately, and support care transitions in partnership with the clinical team.A safety-first foundation. Working knowledge of ADLs/IADLs, safe transfers and mobility support, fall prevention, dementia/behavioral health considerations, infection prevention, and participant rights.Proven people leadership skills. Experience coaching and training staff, managing schedules/coverage, and leading performance management through clear feedback, documentation, and corrective action when needed.Clear, confident communication. Strong collaboration skills with the ability to partner with RN/Provider/IDT, support care coordination, and de-escalate concerns with participants, caregivers, and team members.Operational strength and documentation excellence. Highly organized with the ability to maintain accurate records (EVV/visit verification/timekeeping, service notes, incident reporting), follow policies/workflows, and ensure timely, complete documentation.Comfort with tech and new tools. Proficiency with EMR and scheduling/timekeeping systems, plus email and Slack/Teams; able to learn new platforms quickly and use basic reporting tools.Respect for confidentiality. Ability to handle sensitive information in alignment with HIPAA and organizational standards.CPR/First Aid prepared. Current CPR/First Aid certification required or ability to obtain within the required timeframe (per policy).Language skills that build trust (preferred). Bilingual English/Spanish preferred (or other languages based on participant population).Physical readiness for in-home care. Ability to meet the physical requirements of the role, including frequent standing/walking, occasional lifting, and supporting safe mobility practices in participant homes.Pertinent information:Your base is Monday–Friday—with room for real life. This role is primarily Monday through Friday, and because home care doesn’t always follow a perfect calendar, a little flexibility goes a long way. Expect an occasional early start, later finish, or weekend touchpoint when the field needs coverage—and yes, you may jump into a visit when it helps prevent missed care.You’ll be out and about (not stuck behind a desk). If you like being in motion, you’ll fit right in. You’ll travel locally to participant homes and community locations, so you’ll need a valid driver’s license, reliable transportation, and comfort working across different settings throughout the day.You’ll lead a small but mighty crew.
