Job Description Opportunity Overview Our organization is in search of a committed and empathetic Physician for Skilled Nursing Facilities to enhance our post-acute care team. This role emphasizes delivering exceptional, patient-focused medical services to individuals in skilled nursing and rehabilitation settings. The ideal candidate will possess experience in managing patients with complex medical needs, particularly in post-acute and long-term care, and will demonstrate a strong dedication to continuity of care, teamwork, and improving patient outcomes during care transitions. Key Responsibilities: * Conduct thorough medical evaluations and ongoing care for patients admitted to skilled nursing facilities for post-acute, rehabilitative, and long-term treatment. * Oversee and coordinate care for patients with multiple chronic conditions, acute illnesses, and intricate medication plans. * Perform daily or scheduled visits in designated skilled nursing facilities, ensuring prompt assessment and management of any acute changes in patient conditions. * Work collaboratively with facility personnel, nursing staff, rehabilitation experts, social workers, and case managers to provide integrated, patient-centered care. * Effectively communicate with families, caregivers, and facility staff about patient objectives, treatment strategies, and transitions in care. * Ensure adherence to all skilled nursing facility regulations, documentation standards, and quality metrics, including participation in care plan meetings and quality improvement initiatives. * Collaborate with hospitalists, primary care providers, and specialists to facilitate seamless transitions of care and minimize hospital readmissions. * Engage in quality improvement, population health, and value-based care initiatives related to post-acute and long-term care. * Maintain precise and timely records in the electronic medical record (EMR) system, complying with organizational and regulatory standards. Qualifications: * Medical degree (MD or DO) from an accredited institution. * Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification is preferred). * Active and unrestricted medical license in California. * Proven experience in post-acute, skilled nursing, long-term care, or geriatric environments. * Strong clinical abilities in managing patients with complex, multi-morbid conditions. * Exceptional communication and interpersonal skills, with a focus on patient and team-centered care. * Ability to thrive in an interdisciplinary, facility-based setting. * Familiarity with electronic medical records (EMR) systems. Preferred Qualifications: * Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine. * Understanding of CMS regulations, SNF billing and documentation requirements, and quality metrics. * Experience in value-based care or population health initiatives.
