MAIN DUTIES Carrying out functional assessments of clients’ physical / psychological disabilities considering the limitations imposed upon their self-care, work, home and social life. To provide advice and recommendations to clients and families regarding the management of disability, mobility and all daily living activities and the restoration of these functions if appropriate, by immediate advice or referral to other disciplines. To carry out above duties as part of a multi-disciplinary team having cases referred from multiple sources. Work co-operatively with both internal and external colleagues across multi - agency boundaries. Be responsible for assessing and ordering the most appropriate equipment via the Community Equipment Service to meet the clients assessed need. To review the equipment is suitable for the clients assessed needs once delivered to the client. Be responsible to the Authority for the initial assessment of clients’ needs for adaptations and moving and handling with reference to the various Acts and legislation governing responses. Liaise with Officers from the Home Improvements Service, Registered Social and Private Landlords, Architects and Building Contractors when necessary for the provision of adaptations within dwellings, considering the practicality, the medical condition of the client and the financial involvement to the Authority. Occupational Therapists lone work in the performance of the above duties and are accountable to the Occupational Therapy and Sensory Lead Practitioners and Team managers for the day-to-day management of that service. The above named duties are intended as a guideline for the Occupational Therapists, who will also be required to carry out any other duties or tasks across the Borough and will work to Codes of Conduct, Standards, Policies and Procedures laid down by Health and Care Professions Council, Sefton Council and the Occupational Therapy and Sensory Team Manager. Main Accountabilities Occupational therapist to be responsible for accurate, sensitive, timely and up to date data entry on all relevant databases, considering any legal and human rights of the individuals concerned. About The Role\nThe Support Worker will provide a structured, personalised resettlement service to every service user at Mildmay House. To seek to support service users who have experienced homelessness to move into their own homes in a planned and supported way.\n\nTo offer advice, training, information and one-to-one/group support to help service users through this process. Most new assessments are undertaken by other members of the MDT, but some will require initial medical assessment. There are no fixed requirements for new assessments and follow up appointments. On average there maybe 1-2 new assessments per week and 15-20 follow up appointments, depending on the complexity of the cases and other commitments., • The postholder will be required to participate in the daytime (9am – 5pm) medical on call rota for 1-2 days per week. The initial duty response (including hospital liaison assessments) is made by the duty worker who will contact the doctor on call if required. With an expansion of our crisis services to cover the out of hours period there may be a requirement to provide medical cover for this in the future. Annual leave, Study leave and Job Plan • Consultants are entitled to 30 days of annual leave increasing to 32 days after 7 years’ service (at Consultant level) plus bank holidays., • Study leave for the purpose of professional development should be applied for through the Clinical Lead and is approved by the Director of Medical Education. There is a generous study leave budget and there is an entitlement of 30 days every 3 years., • All consultants have a 10 PA contract for public work, which is non-negotiable. The final job plan will vary according to the successful appointees and be agreed by the Clinical Lead and Chief of Service and ratified by the Group Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance., • All timetables will adhere to current Royal College Guidelines, namely:- 7.5 clinical programmed activities (PAs) 1.5 - 2.5 supporting professional activities (SPAs) consisting of: • 1 SPA for clinical learning and preparation for appraisal (including audit, weekly teaching sessions, case presentations, mortality reviews etc), • 1 SPA for managerial activities, service development and quality improvement (subject to agreement), • 0.5 SPA for other SPA activities tba Some SPAs will be annualised rather than represented in the weekly timetable.
