This role is responsible for coordinating services like meal deliveries, caregiver visits, and medical equipment drop-offs, while also conducting home safety evaluations and supporting care planning. You’ll advocate for participant needs, ensure services align with care goals, and collaborate closely with nurses, social workers, and other team members to improve outcomes for seniors receiving care at home.
