Job Description Home Care Coordinator Location: Chula Vista, CA (Onsite) Schedule: Monday – Friday | 8:00 AM – 5:00 PM Pay Range: $22.39 – $24.00 per hour depending on experience Employment Type: Full-Time Position Overview A respected healthcare organization in South San Diego County is seeking a Home Care Coordinator to support frail and elderly participants through high-quality, person-centered home-based care. This role is ideal for a CNA/HHA who enjoys a balance of care coordination, home visits, and collaboration with an interdisciplinary healthcare team. You will play a key role in ensuring participants receive approved services safely, timely, and in alignment with their individualized care plans. Why You’ll Love This Job • Weekday schedule only, • Meaningful, mission-driven work supporting seniors in the community, • Varied workdays – a mix of coordination, documentation, and home visits, • Team-basedenvironment with strong nurse and interdisciplinary support, • Stable, full-time role with competitive pay, • Hands-on impact – you’ll see the difference your work makes every day Key Responsibilities Key Responsibilities • Coordinate IDT-approved services, including: Meal delivery Durable Medical Equipment (DME) Incontinence supplies Caregiver services • Conduct home safety evaluations (pre-enrollment, annual, semi-annual, and as needed), • Complete and update Home Care Coordination care plans, • Perform home visits for homebound and RCFE participants, including monthly visits, • Monitor service delivery and review progress notes to identify gaps in care, • Document services, actions, and updates accurately in the electronic medical record (EMR), • Upload reports, vendor documentation, and care-related records, • Communicate with participants, families, caregivers, vendors, and healthcare providers, • Advocate for participant needs, preferences, and quality of care, • Assist nurses with coordination of services and supplies, • Deliver supplies, equipment, grocery bags, and other requested items, • Participate in interdisciplinary team meetings, staff meetings, and trainings Required Qualifications • Active CNA certification, • California Department of Public Health Home Health Aide (HHA) certificate, • CPR & First Aid certification (American Heart Association), • Minimum 1 year of experience working with the frail or elderly population, • Valid California Driver’s License, • Strong understanding of HHA scope of practice, • Clear communication skills and ability to work in a team environment, • Basic computer skills (Outlook, Word, Excel) Work Environment & Requirements • Onsite role with frequent community and home-based visits, • Pre-employment requirements include physical, background check, references, and orientation, • Compliance with HIPAA, OSHA, infectious disease, and safety policies, • Required health screenings and vaccinations, • Ability to lift, carry, and deliver supplies and equipment as needed Apply now if you’re a compassionate CNA/HHA looking for a stable, weekday role where you can make a real impact in seniors’ lives. • Active CNA certification • California Department of Public Health Home Health Aide (HHA) certificate • CPR & First Aid certification (American Heart Association), • Active CNA certification • California Department of Public Health Home Health Aide (HHA) certificate • CPR & First Aid certification (American Heart Association) • Minimum 1 year of experience working with the frail or elderly population • Valid California Driver’s License • Strong understanding of HHA scope of practice • Clear communication skills and ability to work in a team environment • Basic computer skills (Outlook, Word, Excel)
