Key Responsibilities: • Conduct comprehensive mental health assessments, diagnoses (using DSM-5-TR criteria), and develop individualized treatment plans., • Provide evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), Solution-Focused Therapy, and Trauma-Informed Care., • Facilitate individual, group, and family therapy sessions to address a variety of mental health conditions including depression, anxiety, trauma, substance use disorders, and severe mental illnesses., • Collaborate with psychiatrists, primary care providers, and other members of the interdisciplinary team to ensure integrated care., • Engage in case management activities, linking clients with community resources such as housing, employment services, support groups, and social services., • Maintain accurate, timely documentation of all clinical interactions in accordance with legal, ethical, and agency standards., • Utilize risk assessment tools to evaluate and respond to suicidality, self-harm, violence risk, and other psychiatric emergencies., • Provide crisis intervention services as needed, including safety planning and referrals to higher levels of care., • Educate clients and families about mental health conditions, treatment options, coping strategies, and recovery supports., • Participate in clinical supervision, ongoing professional development, and agency trainings to maintain licensure and enhance clinical skills., • Uphold the highest ethical standards as outlined by the National Association of Social Workers (NASW) Code of Ethics. Concerto is hiring a salaried On-Call Dialysis Nurse who will be on call five days a week (Monday - Friday + 2 Saturdays per month), with actual workdays varying based on coverage needs and eligible for overtime. The technician will be assigned to a specific region and must travel within and occasionally outside that region, receiving additional base pay for out-of-region travel and mileage reimbursement for all travel from their home clinic to the work site. This role involves working under the supervision of a licensed nurse to provide safe and effective dialysis therapy in compliance with Concerto’s policies and state and federal regulations.
