We're seeking an experienced Occupational Therapist to join our full-time Home & Community Neurorehabilitation team in San Jose, CA, delivering meaningful, functional care beyond the clinic walls. In this role, you'll serve as a primary therapist from evaluation through discharge, helping patients rebuild independence through real-life activities that matter most. Who We're Looking For: • An experienced Occupational Therapist ready to manage the full episode of care from evaluation to discharge., • Comfortable with a 40-hour salaried caseload, typically 5-7 patients per week (most often 5 patients receiving approximately 6 hours of treatment each)., • Passionate about functional, real-world rehabilitation delivered in homes and community settings., • Motivated to help clients successfully transition back to everyday life, including:, • Independently picking up and managing prescriptions, • Safely returning to driving, • Transitioning back to work, • Participating in yoga, light hiking, and community activities, • A clinician who thrives with autonomy while valuing collaboration with a dynamic interdisciplinary team of OTs, PTs, SLPs, and other specialists.Why Rehab Without Walls®: We bring the outpatient experience to the individual, delivering high-touch, one-on-one care in real environments allowing patients to practice skills exactly where they're needed. Responsibilities What you will do: Responsibilities listed include but not limited to: • Perform evaluations and develop treatment plans, • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences, • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources, • Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members What you will need: • Minimum of a bachelor's degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program, • Current, unrestricted license as an Occupational Therapist by state in which practicing, • Current CPR Certification, • A minimum of one year's work experience as an Occupational Therapist Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care. Additional Job Information • Communicates effectively and professionally in verbal and written interactions, • Ability to lift 50 pounds, • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures, • Duties require fine motor skills,visual acuity, and walking/ standing for extended periods, • Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times USD $53.00 - $60.00 / Hour
