JOB SUMMARY/ESSESNTIAL DUTIES AND RESPONSIBILITIES: • Review and abstract data from electronic health records (EHR) and paper records to meet core measures, regulatory, and accreditation requirements, especially those related to stroke care. This includes information on diagnoses, treatments, procedures, and outcomes, • Maintain up-to-date knowledge of TJC, CMS, and other quality program guidelines and ensure abstraction criteria align with current specifications, • Adheres to specific clinical data abstraction guidelines and standards, such as those set by the American Heart Association, • Enters the abstracted data into a database or NCDR registry, often using specialized software. This includes maintaining and updating the data as necessary, • Participates in quality assurance processes to ensure data integrity. This involves regular audits of the data or the abstraction process, • Remains informed about developments in cardiac care and data management to ensure ongoing competency in the role REQUIRED QUALIFICATIONS: Education: • Bachelor’s degree in nursing, Health Information Management, or related healthcare field preferred, • 2–4 years of experience in a hospital or healthcare setting, preferably in quality, case management, HIM, or clinical documentation, • Experience with Joint Commission and Get With The Guidelines Stroke and Comprehensive Stroke within the last 2 years preferred, • Prior experience with core measures abstraction strongly preferred., • Familiarity with CMS/TJC specifications, ICD-10, CPT, and medical terminology, • Certifications (preferred, not always required): RN, BSN, MSN CPHQ, or related KEY SUCCESS ATTRIBUTES: • Integrity, passion, and ethics are required, • Demonstrates strong collaboration skills, • Has strong analytic and problem-solving abilities and techniques, • Exhibit consistent initiative with strong drive for results and success, • Demonstrate commitment to a team environment, • Well-developed written, verbal, and presentation communication skills including deep listening and attention to detail, • Ability to self-motivate and self-direct, • Excellent organizational, analytical, and communication skills, • Commitment and adherence to company Core Values, • Strong attention to detail and ability to interpret clinical documentation, • High level of integrity & ethical judgement, • Maintains a strict level of confidentiality in all aspects of work, • Communication, • Consistency and Reliability BENEFITS: We offer an excellent salary, full benefits package including 401(k) with company match, medical, dental, vision, life, short/long term disability insurance, and PTO policy. PHYSICAL DEMANDS OF THE ESSENTIAL FUNCTIONS: Sitting, talking, hearing and near vision are required over 90% of the time. Feeling is required over 90% of the time and reaching is required about 50% of the time. The ability to travel to field sites may be required up to 15% of the time. WORKING CONDITIONS WHILE PERFORMING ESSENTIAL FUNCTIONS: Over 90% of the time is spent indoors, with protection from weather conditions. Exposure to noise levels that may be distracting or uncomfortable is present in only unusual situations.
