We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to serve as our Health & Wellness Director. As our Health & Wellness Director, you will lead the care team, oversee resident wellness, and partner with families, physicians, and staff to ensure exceptional care for every resident while supporting compliance with Colorado Assisted Living Residence (ALR) requirements. Position Summary The Health & Wellness Director is responsible for overseeing the daily clinical operations of our Assisted Living and Memory Care community. Working under the direction of the Assisted Living Director and in collaboration with the community's healthcare providers, this position provides leadership to the Qualified Medication Administration Personnel and caregiving teams while ensuring residents receive safe, compassionate, person-centered care. This role is responsible for resident assessments, service planning, medication management oversight, staff training, documentation, incident follow-up, and regulatory compliance in accordance with Colorado Assisted Living Residence (ALR) standards and community policies. Essential Responsibilities • Provide leadership and daily oversight of the nursing and caregiving teams., • Promote a culture of compassion, accountability, teamwork, and exceptional resident care., • Complete resident assessments prior to move-in and on an ongoing basis as required by resident condition and community policy., • Develop, review, and update individualized service plans and care coordination needs., • Monitor changes in resident conditions and communicate promptly with physicians, families, hospice providers, pharmacies, and other healthcare partners., • Oversee medication administration, medication assistance, and medication management processes in accordance with Colorado ALR regulations and community policy., • Ensure accurate, timely, and complete clinical documentation, including assessments, service plans, medication records, incident reports, and follow-up notes., • Maintain compliance with Colorado Assisted Living Residence regulations, resident rights requirements, and community policies and procedures., • Support admission, retention, and discharge processes by evaluating resident needs and coordinating appropriate care planning., • Recruit, train, coach, and support Qmap and caregiving staff., • Provide ongoing education to staff on resident care, infection prevention, emergency procedures, documentation, resident rights, and medication practices., • Conduct clinical audits and participate in quality assurance, infection prevention, and corrective action initiatives., • Respond to resident emergencies and provide clinical guidance when needed., • Participate in resident care conferences and interdisciplinary meetings., • Build trusting relationships with residents and families through compassionate communication and responsive service., • Support staffing and scheduling needs to help ensure resident care needs are met., • Collaborate with outside healthcare providers and consultants to support resident wellness and regulatory compliance.Qualifications, • Current Colorado Licensed Practical Nurse (LPN) license in good standing., • Minimum of two (2) years of nursing experience., • Previous leadership or supervisory experience preferred., • Experience in Assisted Living, Memory Care, Long-Term Care, or Senior Living preferred., • Strong knowledge of resident assessments, service planning, medication management, documentation standards, and Colorado Assisted Living Residence (ALR) regulations., • Familiarity with resident rights, incident reporting, infection control, and care coordination requirements., • Excellent leadership, communication, and organizational skills., • CPR certification or ability to obtain upon hire., • Ability to work collaboratively with residents, families, staff, physicians, and outside providers., • A genuine passion for serving older adults with dignity, patience, and compassion.What We're Looking For Our ideal candidate is someone who: • Leads with integrity and compassion., • Is comfortable coaching and developing others., • Communicates effectively with residents, families, physicians, and team members., • Is organized, dependable, and solutions-focused., • Understands that outstanding senior care begins with meaningful relationships., • Takes pride in creating a warm, welcoming environment where residents feel valued and cared for., • Is comfortable working in a regulated environment and supporting compliance with Colorado ALR expectations.Schedule Full-Time Generally Monday through Friday during regular business hours, with flexibility to meet the needs of the community.
