WHAT YOU WILL DO TO FURTHER THE BIAC MISSION: The Director of Resident Services will oversee the Permanent Supportive Housing (PSH) model and work with the on-site Valor on the Fax team to provide support through resident/family advocacy, service coordination, care planning and programming. This position works with other staff members to incorporate person-centered practices with a trauma-informed approach to create a safe environment for residents and community engagement. Valor on the Fax is a 72-unit permanent supportive housing building for persons experiencing homelessness who also have a brain injury or related disability. ESSENTIAL DUTIES/RESPONSIBILITIES: * Oversee the work related to PSH and the Housing First Model to ensure high quality resident care. The Registered Nurse – Care Coordinator at Stout Street Health Center (SSHC) is a key member of the Colorado Coalition for the Homeless (CCH) team, dedicated to delivering high-quality, compassionate healthcare to homeless and at-risk individuals. In this role, you will apply your clinical expertise within an integrated, trauma-informed care model, working closely with a multidisciplinary team to address the complex health needs of our diverse patient population. Your commitment to patient-centered care, coupled with a deep sense of empathy and social responsibility, will drive your efforts to empower patients and improve their overall well-being. This position requires adaptability, resilience, and a proactive approach to overcoming the barriers that often limit access to healthcare for the most vulnerable members of our community.
