Job Description On Site Location: Bulverde, TX PRIMARY FUNCTION/RESPONSIBILTY The Case Manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations, while maintaining compliance with established state and federal standards. The Case Manager shall possess experience in working with children and families. The ideal employee will have excellent oral and written communication skills and should be able to gain the trust of youth and maintain a safe living environment. ESSENTIAL DUTIES AND RESPONSIBILITIES • Develops, assesses, facilitates, and monitors all aspects of assigned caseloads., • Participate in youth intake and assessment interviews., • Maintains youth files, collaborates with Treatment Director/therapist to develop goal-oriented child plans of service, and maintains confidentiality in all youth and program operations and communications., • Uses electronic medical record, both on-site and remotely, to compile, maintain, and distribute appropriate documentation., • Establishes supportive and collaborative relationships with youth, families/guardians and, as applicable, external stakeholders., • Prepares and submits documentation, including assessments and other such reports within required timelines., • Discusses with assigned youth, on a weekly basis, the circumstances key aspects of the Integrative Service Plan including future placement options, daily living circumstances, and other post-discharge services., • Provides crisis intervention for youth and families., • Attends and participates in training, workshops and in-services provided by Touchstone Alliance or outside agencies as required., • Maintains professional and ethical standards as documented by Touchstone Alliance and serves as a positive, professional role model., • Complies with Touchstone’s written policies and procedures and adheres to all applicable personal and organizational licensing standards and funding source guidelines., • Participates in the development of assessment, service delivery, discharge, and aftercare plans., • Reports any/all suspecting abuse and neglect to the Administrator on duty who will make and immediate report to DFPS at txabusehotline.org., • Administers and documents the delivery of all prescribed medications., • Provides case management services to youth and assists staff with GRO compliance., • Ensures regular contact between youth and family, when applicable, and monitors psychological, mental, emotional, physical, environmental well-being through youth’s communications., • Provides resource information and referrals to community organizations for evidence-based therapeutic interventions., • Performs on-site crisis intervention to prevent escalations of high-risk behaviors, relapses, hospitalizations, placement disruptions, and other negative outcomes., • Provides "PRN" (as needed) services including 24-hour awake, continuous coverage to meet the particular needs of a youth., • Consults with Treatment Director and other specialists, and/or treatment team members concerning plans of care and makes recommendations as needed., • Ensures that medical and psychiatric appointments for diagnostic assessments are scheduled and completed according to state regulations., • Obtains external medical and dental records and makes appointments as necessary., • Assist with home, campus, and classroom maintenance and upkeep., • Interfaces with other staff and childcare professionals as a team player., • Perform other related duties as required.Case Manager - Aftercare Responsibilities:, • Develop and implement comprehensive discharge planning from the start of placement, incorporating input from the Service Planning team, family members, and community providers., • Ensure assessments are conducted as needed and maintain detailed documentation of child and family progress., • Facilitate monthly face-to-face, phone, or virtual contacts with children and caregivers for at least 6 months post-discharge., • Coordinate educational transitions, including school enrollment and maintaining Education Portfolio., • Submit and track community service referrals to ensure continuity of care in areas such as mental health, medical care, and social services., • Support families in developing strong support networks and accessing community resources, • Organize and facilitate team meetings every 30 days post-discharge as needed., • Assist in transition planning specific to placement type (new provider, reunification, guardianship, adoption, or independent living).QUALIFICATIONSRequired, • Bachelor’s degree in human services or related field., • Valid Texas/State driver's license, appropriate insurance, and reliable transportation, • Have or be able to complete CPR Certification and renewals, • Willingness to undergo random drug tests during employment, • Age minimum of 21 years of age per licensing standards.PHYSICAL/MENTAL REQUIREMENTS AND INFORMATION, • Body mobility to stoop, kneel, bend, reach, and walk and walk briskly in order to interact with and monitor children., • Stamina to work long days and drive long distances., • Moderate to heavy lifting to up to 60 pounds., • Must have the ability to communicate both verbally and in writing., • Must have the ability to give and receive verbal and written instructions (with or without hearing aids)., • Must have the ability to read fine print, have sustained vision, and peripheral vision (with or without glasses)., • Environment requires dexterity to remain calm and adapt to fast paced and emotional environments consisting of children with moderate to severe emotional and psychological needs., • Home-like settings with varying moderate to high degrees of background noise., • Light and ventilation are found in a typical home setting., • Campuses are smoke-free, except on permitted campuses in designated areas., • Full-time position with minimum 40 hours. Campus setting with 24/7 residential operations and 24/7 on-call responsibilities. Supervises: N/A
