The Westchester County Department of Public Health has an immediate opening for a Breastfeeding Peer Counselor to provide basic information and encouragement to pregnant and breastfeeding mothers. This is a part time paraprofessional position based primarily at the WIC Program, Yonkers site. Summary of Position: PHS is seeking a health coach to fulfill the following duties: • Provide health education and home visiting services to pregnant participants throughout pregnancy and through 12- months postpartum., • Facilitate curriculum-based group health education workshops and support enrolled participants in the CHIME study with maternal & child health resources in their community., • Responsible for working with participants on their nutrition, physical activity, and lifestyle goal setting, as well as a detailed social needs screening, assessment, and referral., • Meet with participants on a weekly basis (at their home, in the office, or in the community) and keep detailed notes on their progress and activities, providing in-person support, text-message resources, and referrals to community services., • Manage caseload of up to 30 participants at a time and meet all stated program goals., • Follow guidelines from the Health Coach Manual on proper and in-depth case note documentation and tracking participant enrollment and progress in the study., • Regularly collaborates with program manager, study primary investigators (PIs), research coordinators, other team members, and nutrition/physical activity consultants to troubleshoot adverse events that may arise or discuss participants. The Manager of Peer Recovery Support Services also functions as the Patient Navigator / Trainer within the OORP program. Job Duties 1. Embodies Eva’s Village’s core values, voice, and tone to provide an exceptional and effective experience for our clients, funders, employees, and volunteers., 2. Oversee, supervise, and monitor the daily operations of the RCC and OORP program, services, and staff., 3. Collaborates with the Senior and Associate Peer Services Coordinators to develop and coordinate activities and services within the Recovery Community Center, ensuring alignment with related programs, departments, and staff to maximize efficiency and maintain compliance with relevant policies, procedures, and standards., 4. Shall attend any training and/or meetings in person or via conference call that are scheduled by DMHAS to support the delivery, sustainability, and effectiveness of OORP services., 5. Maintains effective intra-office and inter-office communication within departments., 6. Shall ensure cooperation and collaboration with the DMHAS statewide evaluator for the OORP, which may include focus group participation; key informant interviews; and individual data reporting (i.e., select social indicators; substance use, overdose and mental health history; patient outreach; services provided, and recovery and treatment follow-up)., 7. Data shall consist of a minimum of the following: · Number of reported opioid overdose reversals by Emergency Department. · Basic demographics on the individual reversed from an opioid overdose, including a minimum of the following: Age, Race, Hispanic origin, Gender · Number of visits per month by the Recovery Specialists, Number of successful referrals to treatment by Level of Care (LOC) · Number and types of referrals to recovery supports other than substance abuse treatment (i.e. mental health services, housing, employment, etc.) · Number of follow-up phone calls by Recovery Specialists per month 9. Shall submit a Narrative Contract Progress Report to the Senior Director of Integrated Services and/or Clinical Administrator for review before submission to the DMHAS Project Director within 5 working days of the end of each quarter., 10. Maintain client information and ensure up-to-date documentation in the OORP Teams Channel., 11. Providing an evidence-based comprehensive case management assessment that includes life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs., 12. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment, 16. Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Position Summary: Under the general supervision of the ABD Sr. Program Manager and Director of Health Insurance Enrollment, Home Visitor Facilitated Enrollers will engage the aged, blind, and disabled (ABD) populations through home visits, outreach, and at community-based sites. This role will engage the ABD populations who are homebound and assist clients with the application process for public health insurance through home visits 5 days a week (including weekends per request) across the 5 NYC boroughs. Home Visitor Facilitated Enrollers will educate consumers about insurance options, benefits, length and terms of coverage and application process as well as about managed care, health plans, and primary/preventive care. They will also provide guidance to consumers on required documentation and assist consumers with health insurance applications. Schedule may vary due to program needs JOB SUMMARY: Provide high-quality, culturally competent child development and parent support services with an emphasis on the role of the parent as the child's first and most important relationship.
