Must be able to stabilize and promote self-sufficiency and optimum health for people living with HIV/AIDS, Mental illness and/or substance use disorders. Manage a case load of 20 to 25. SPECIFIC DUTIES & RESPONSIBILITIES: • Provide substance abuse intervention, counseling, educational services, and case management services to tenants., • Evaluate and recommend treatment plans for tenants with mental health and chemical dependency problems after a one-to-one interview., • Provide regular, individual, and group counseling services., • Establish community and agency affiliations to strengthen the delivery of services for tenants, including medical & detoxification when necessary., • Attend case review conferences with HRA/HASA and provide written evaluations regarding tenant’s drug problems and money management issues., • Present case load weekly at chart review., • Meet for supervision with Assistant Director bi-weekly., • Orient tenant’s to the program upon admission., • Maintain up-to-date tenant’s charts, case notes, service plans and psychosocial., • Attends all in-service training., • Responsible for collection of tenants monthly rent., • Set a relaxed tone and social environment (by role modeling), • Other duties as assigned REQUIREMENTS: EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES: High School Diploma, one to two years of experience in counseling or two (2) years of Odyssey House experience and a High School Diploma, or Bachelor’s Degree in Human Service. Description of Duties and Responsibilities. • Engage PWUD and community members in parks, encampments, transit hubs, shelters, and other high-need areas., • Distribute naloxone, fentanyl/xylazine test strips, sterile supplies, first aid kits, hygiene kits, and other harm reduction materials., • Provide overdose prevention education and train individuals and families in naloxone administration., • Offer peer support, motivational interviewing, and trauma-informed engagement strategies to build trust., • Support clients with navigation of Medicaid, housing, benefits, and treatment referrals, including warm handoffs and accompaniment to appointments., • Conduct field-based behavioral health screenings (e.g., PHQ-9, GAD-7) and refer clients to appropriate providers., • Maintain accurate outreach records and ensure timely documentation of encounters, referrals, and outcomes., • Collaborate with community partners to expand access to harm reduction and treatment services, • Participate in ongoing training, group supervision, and reflective practice sessions. Skills and Knowledge Required: Strong skills in trauma-informed care, harm reduction practices, overdose prevention, and motivational interviewing are essential, along with the ability to engage clients with empathy and respect. Instructs, assess and monitors patient’s outcomes., • Assess/monitor/revises changes in occupational therapy plan for effective patient care., • Coordinates with field staff and develops patient care plans to meet identified occupational therapy needs, seeks clinical guidance from occupational therapy managing personnel to service the hard to service patients., • Evaluates and treats cognitive perceptual defects., • Develops implements patient specific Home Exercise Program and provides timely upgrade to this Program., • Timely documentation of patient care information on evaluation data, treatment modalities, communications on patient program and response to interventions in patient clinical records., • Provides patient/caregiver/healthcare worker education on Home Exercise Program, adaptive equipment/device/ monitors/assess for understanding, compliance and outcomes., • Maintains records for each patient treated; by documenting findings, diagnosis, treatment methodologies, interventions, special precautions, progress notes, record of visits and discharge summary., • Ongoing communication with the HHA nurse regarding all information; assessment findings, barriers that will influence or change provision of services including any unusual situations observed during the course of service., • Provide timely reports, outcomes, recommendations and changes in the care plan to the patient’ physicians., • Participates in Agency in-service education programs, Quality Assurance, Interdisciplinary Team Meetings., • Consults with other members of the health team to coordinate therapeutic programs for all patients including the hard to service patients., • Timely documentation and submission of patient clinical findings, treatments/interventions outcomes and plans., • Initiates and maintains verbal/written communication according to the HHA policy to ensure coordinated patient quality care., • 60 Day Reports: Re-evaluates patients every 60 days to detect outcomes/progress attained and documents findings, discharge plans and changes to the care plans for the upcoming 60 Days. Submits reports within three (3) business days following the date for inclusion in the patient’s clinical record., • Discharge Summary: Submits discharge summaries at the completion of services that includes assessment findings, treatment methodologies, service outcomes and any recommendations for follow-up care., • Assumes responsibility for continued professional growth by maintaining professional memberships and annual professional certifications., • Performs other related duties as required.

MAJOR FUNCTIONS: The Discharge Coordinator is responsible for guiding consumers and their families through the health care and supportive housing system by providing referrals, assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Discharge Coordinator assumes a central role in ensuring community based services and supports are available for those consumers participating in the Reintegration phase and/or prior to discharge. S/he works directly with consumers in individual and group sessions on issues surrounding discharge planning, identifying community based supports and securing appropriate living arrangements. SPECIFIC DUTIES & RESPONSIBILITIES: • Monitor and evaluate consumer needs, including prevention, wellness, medical, mental health, care transitions, and social and community services where appropriate., • Maintain up-to-date consumer charts including referrals and outcomes., • Support adherence to treatment plan recommendations, • Responsible for ongoing consumer discharge planning., • Review and generate daily referrals., • Coordinate schedules, troubleshoot conflicts and ensure compliance with all appointments for designated consumers., • Develop and facilitate Independent Living Skills workshops to build skills and increase knowledge related to activities of daily living, ability to access community-based resources, spending habits and money management., • Assist consumers in navigating the subsidized housing identification and application process. This includes completing 2010e applications on all residents seeking subsidized housing., • Assist consumers in filling out all necessary paperwork, accompanying residents to interviews, following up with supportive housing agencies and ensuring that all housing units applied for are safe and affordable., • Link consumers to instrumental support services such as Medicaid, Medicare, Social Security, food stamps, home health care, Senior Citizen Rent Increase Exemption (SCRIE) and home delivered meals prior to discharge., • Link consumers to psychosocial/healthcare support services (e.g. SUD recovery supports, aftercare, mental health, primary care, social adult day services, senior centers, etc.) prior to discharge., • Attend regularly scheduled staff meetings and case conferences., • Attend all required in-service training seminars., • Participate in quality improvement activities., • Other relevant duties as assigned. REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES • Bachelor’s Degree or Associate’s Degree with two (2) years of related experience in health insurance, care coordination, entitlement, or housing placement services for low income and/or homeless populations., • Experience providing service coordination and information, linkages, and referrals for community-based services., • Knowledge of city, state, and federal guidelines for public and private low-income housing, entitlements and health insurance., • Ability to research and network with other agencies/community based services to meet a variety of client needs., • Proficiency with computer operation (Microsoft Word, Excel, AWARDS, and Outlook programs)., • Excellent written and verbal communications skills., • Must be able to work a flexible schedule., • This position requires regular travel throughout the five (5) boroughs.
