Job Description POSITION STATEMENT: The Program Specialist will: • Function with autonomy, diplomacy and professionalism within the guidelines established by this organization, • Promote the well being of the human aspect of this organization and report any concerns to the Executive Director, • Function with the highest esteem in dealing with all employees, supervisors, and consumers of this organization, • Be an advocate for all of the policies and protocols of the organization RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS: • Maintain a caseload of no more than 30 individuals, • Complete quarterly reports, annual assessments, outcomes, SNAP assessments, personal inventories, consumer demographic sheets and required paperwork for the 6400 regulations and PA 51 regulations for all individuals on caseload, • Attend monthly Waiver monitoring meetings and complete follow up actions for Waiver monitoring issue forms, • Provide needed documents to the Supports Coordinator, for individual’s meetings as well as attend meetings with other team members, • Function as the initial point of contact for Supports Coordinators, Administrative Entities, and Government agencies, • Visit all homes on the caseload a minimum of twice per month to include one awake night. More visits will be necessary if individuals are in crisis, new to programming, or have increased training needs, • Participate in monthly Team meetings for assigned team and homes, • Attend all ISP meetings, complete annual outcomes/procedures and all needed paperwork and submit to the Supports Coordinator, • Ensure the resident’s home is a safe, functional living space to meet individual needs, • Participate in medical appointments (especially psychiatric or hospital discharge) as needed to ensure additional services are requested if needed and that information is relayed appropriately, • Participate in on-call rotation, as assigned, • Create, monitor, and update house books for all individuals on caseload, • Coordinate and participate in all evaluations including, PT, OT, speech, sensory, and any other required support, • Work in conjunction with Referral Team on coordination with pending placements, as well as with the families and present provider of these individuals, • Coordinate and participate in all evaluations concerning wheelchairs, medical and adaptive equipment, • Participate in outside reviews i.e. SIS assessments, quality assurance visits, and other visits as scheduled, • Review all ISP’S for individuals on case load and ensure that the Assistant Program Coordinator administers competency tests to all employees on new plans, • Notify appropriate Supports Coordination entity of errors or inconsistencies in ISP hours or needs and coordinate with Central Scheduling, • Attend staff meetings and training sessions as required, • Review noted events and ensure incidents submitted timely, • Completion of annual consumer inventory. Ensure updates are completed as large purchases or disposals are completed, • Prepare and maintain all required licensing files throughout the year for each resident in preparation for annual licensing and follow up with Program Coordinator/Assistant Program Coordinator for needed items, • Coordinate completion of Quarterly Licensing Inspection summaries as well as Licensing Inspection Instruments, • Draft and update Behavior Support Plans as needed (if not completed by PDCS), • Work in conjunction with Program Coordinators and Assistant Program Coordinators if a crisis arises in the home related to the individuals, • Promote residents, staff and company in a positive manner to neighbors, doctors, families and the public at large, • Maintain and manage Individual record compliance (Setworks) including but not limited to: emergency demographics, annual consent packet, Annual SNAP, HRST, Dental plan, Quarterly reviews, Medical compliance- coordinate & track medical appointments/ paperwork, Enter outcomes in Setworks/ monitor documentation, • Submit EIM Submission/ Finalizations, • Review and participate in medication error debriefing, • Act as SCO liaison, Attend all SC waiver monitoring and communicate updates to ISP, • Participate in all SIS assessments, MA Renewals, SNAP benefit applications, • Act as Family liaison, • Maintain documents in Critical Info Book and sign as the initial trainer for all plans & protocols, • Track/ audit staff training on plans & protocols, • Co-facilitate bi-monthly house meetings, • Manage and maintain refusals, write desensitization plan, enter plan questions related to refusals, and document progress on quarterly report, • Assist with additional duties as assigned by Program Management MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS: • Associates Degree (or 60 credit hours) and 4 years of experience working directly with people with developmental disabilities, • Excellent communication skills, documentation accuracy, and organizational skills Who is Apis Services Inc: Apis Services, Inc.
