Position: Case Manager Reports To: Program Manager Location: 57 East 128th Street, New York, N.Y 10035 What The Case Manager Does: • Review all documentation establishing tenants' eligibility for the program and make file copies., • Assist tenants in completing all CAMBA intake applications and forms., • Create and maintain tenant files., • Conduct initial intake or assessment of tenants and tenants' families and/or periodic reassessments., • Follow-up with tenants and referral organizations regarding tenant contact and progress with referral organization., • Work with tenants to break through barriers to goals and assist them in advocating for themselves and moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits and document via progress notes., • Provide all required information for weekly/monthly/quarterly/annual reports., • Participate in case conferences., • May recommend the closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for months; or (b) have not demonstrated a willingness to participate in the process (lost-to-service); or (c) have become ineligible for services (e.g. moved out of area)., • May act as tenant liaison/tenant advocate with outside organizations regarding education, healthcare, housing, legal issues, etc., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • May prepare marketing materials for the program., • May plan, coordinate, and facilitate social/peer support events, including group facilitation for tenants., • May input tenant data and tenant progress information into the automated database., • May reach out and market the program to the community to recruit tenants., • May prescreen tenants over the telephone for eligibility and may schedule intake appointments., • May escort tenants to appointments (educational, medical, social service, etc.), • May assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf., • May follow up with tenants for a period of time after the successful completion of their primary goals to ensure tenant stability. Job Description Title of Position: Case Manager Department: Transitional Housing Reports to: Social Service Supervisor FLSA Status: Non-Exempt Salary: $25.98 per hour Positions reporting to this position: None Purpose of the Position The Case Manager is responsible for case management services for clients in transitional housing. This role assists consumers in developing and maintaining stable living, working, and social environments by coordinating access to medical, psychosocial, financial, vocational, and other essential services. The Case Manager provides crisis intervention as needed and delivers services in the field to support individuals in achieving the highest level of independent living possible. Primary Responsibilities • Travel to/visit consumers’ residences to provide supportive services on a schedule established by the program; counsel consumers individually or in groups to meet service needs and goals, • Assist consumers in modifying attitudes and patterns of behavior by increasing understanding of self and others, • Secure and evaluate information related to medical, psychological, and social factors; complete and maintain assessments, service plans, treatment records, and documentation (including quarterly reviews), • Maintain accurate, up-to-date case records and measurable service plans in compliance with regulatory standards and culturally responsive practices, • Provide supportive counseling (individual and group) and utilize evidence-based approaches such as cognitive-behavioral techniques, • Obtain, develop, and communicate resource information and connect consumers to appropriate community supports, • Facilitate individualized services aligned with recovery and discharge planning goals, • Educate consumers on available services, recreational activities, and daily living skills, including socialization and conflict resolution, • Perform crisis assessments and provide verbal and non-verbal crisis intervention as needed, • Refer consumers to appropriate community providers and accompany them to appointments, • Participate in on-call responsibilities, if assigned, • Report serious incidents immediately; complete incident documentation and follow agency policies, • Plan and coordinate social and recreational activities, • Conduct regular inspections of consumer residences to ensure safety and cleanliness, • Maintain staff communication logs and documentation, • Execute emergency response protocols and provide first aid/CPR if necessary, • Attend meetings, supervision, and trainings, • Advocate on behalf of consumers internally and externally, • Assist consumers in accessing vocational training, employment, and education opportunities, • Perform other job-related duties as assigned

The project consists of 23 residential units, 5 singles and 18 families with CAMBA, Inc. providing the on-site services Position: Case Manager Reports To: Program Manager Location: 1047 Beach 21st Street, Far Rockaway, NY 11691 What The Case Manager Does: • Maintain professional relationships with clients and client confidentiality., • Practice Universal Precautions/Standard Protocol & Procedures., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • Act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • Schedule appointments for tenant with referral organizations., • Escort tenants to appointments (educational, medical, social service, etc.), • Assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf., • Follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • Evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program., • May prescreen tenants over the telephone for eligibility and may schedule intake appointments., • Input tenant data and tenant progress information into automated database Minimum Education/Experience Required: • Bachelor’s degree (e.g., B.A., B.S.W.), • Experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience. Compensation: 40,000-46,250 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program.
