We are seeking a committed and compassionate Social Work Officer (PO1) to support adults across the borough under a strengths-based model of practice. You will hold a caseload of non-complex cases carrying out assessments reviews and support planning to help residents maximise their independence wellbeing resilience and engagement with community resources. You will work closely with qualified Social Workers and multi-disciplinary teams contributing to improved outcomes and ensuring residents receive the right support at the right time. This role is vital in delivering high-quality adult social care and supporting safe effective and person-centred decision making. • Hold a caseload of non-complex cases supporting adults to improve independence resilience and wellbeing., • Undertake strengths-based assessments reviews and support planning., • Provide practical support including helping service users access: Living aids and equipment Welfare benefits Community resources., • Assist in arranging short-term care and respite to help prevent carer breakdown., • Work with adults families carers and communities to help them make informed decisions and engage in their support plans., • Develop effective working relationships to motivate and support individuals., • Liaise with colleagues across the health and social care partnership., • Attend MDT meetings and contribute towards achieving positive outcomes., • Attend safeguarding strategy meetings as required., • Maintain accurate timely and complete case notes documentation and reports., • Update systems and contribute to information gathering for legal purposes where required., • Undertake regular reviews of care packages., • Complete Continuing Health Care (CHC) checklists and identify cases requiring full assessment., • Genuine motivation to develop a career in Adult Social Care., • Understanding of how Adult Social Care supports vulnerable adults to maximise independence., • Ability to explain and apply social care values ethics and strengths-based practice., • Demonstrated achievement focus and reliability in previous work or voluntary experience., • Strong understanding of diversity equality and cultural awareness., • Competent in using IT systems for case recording and communication., • Enhanced DBS required, • Experience supporting adults in a social care environment (voluntary or paid)., • Knowledge of local community resources and support networks., • Proof of Right to Work in the UK, • Enhanced DBS Check, • Satisfactory references, • Compliance with Data Protection GDPR and safeguarding policies, • Play a meaningful role in supporting vulnerable adults., • Gain hands-on experience in strengths-based practice within a supportive team., • Work collaboratively with social workers MDTs and partner agencies. Required Skills: Person Specification Essential: Minimum 6 months of experience in a similar caretaking or facilities role Knowledge of Health & Safety and COSHH (Control of Substances Hazardous to Health) procedures Ability to work independently follow instructions and manage daily tasks without supervision Good communication skills and a professional approach to duties Reliability punctuality and a strong work ethic Desirable: Previous experience working in a residential school or public-facing environment Basic maintenance or DIY skills Familiarity with safeguarding procedures (training can be provided) Required Education: Level 2 Diploma in Health & Social Care (or equivalent) or willingness to work towards this.
