Gracefield Health Care Limited provides residential services to adults between the ages of 18 and 65 years who have a learning disability and additional complex needs such as autism, mental health and behaviours which challenge services.\n\nKey Objectives\n\nTo play a key role in actively promoting and supporting individuals to live safely and independently within the home and community.\n\nTo assist in the provision of a flexible and personalised service.\n\nTo act as a link between Residents, Families and the leadership team and to promote oneself as an appropriate role model to other team members.\n\nTo promote good working practices at St Domingo House by means of an awareness and understanding of the company’s policies and procedures, as well as the aims and objectives of the company.\n\nMain Duties\n\nThis is not a comprehensive list of all the tasks which may be required of the postholder but is illustrative of the general nature and level of responsibility of the work to be undertaken.\n\nTo carry out all company duties, tasks and responsibilities in accordance with the company’s principles, and philosophy of care, whilst ensuring the safety, care and wellbeing of the residents and colleagues at all times. To carry out the role effectively, ensuring compliance with the values and ethos of the service.\n\nTo support residents on a day-to-day basis ensuring appropriate care and support is delivered and maintained at all times per care plans and risk assessments.\n\nTo assist residents with their personal care, maintaining dignity and privacy. This may include assisting with bathing, washing and helping with the toilet. Support may also be required with dressing, undressing and other care and support tasks.\n\nTo encourage residents to take their medication as prescribed. This may entail administration of medication following all relevant recording processes. To follow the company’s medication policies and procedures to ensure the safety of the individual.\n\nPreparing food in accordance with the individual’s choices, following good food hygiene recommendations. Assist where necessary with eating and drinking if required.\n\nTo support residents to manage their finances in line with the company policy. Ensuring accurately recording all transactions.\n\nTo ensure residents are actively engaged in the key worker process, relationships and taking responsibility and to make sure all paperwork is completed in line with company policy.\n\nTo assist in the preparation and implementation of care plans and risk assessments to evaluate the effectiveness of individuals programs in order to support with measuring progress and to make recommendations to the leadership team for further improvements.\n\nTo contribute to the monitoring of person-centered plans by accurate communication of individuals needs and outcomes achieved to the leadership team.\n\nTo support residents on activities, excursions and holidays. To actively encourage residents to participate in social, educational and leisure pursuits.\n\nTo participate in service user reviews.\n\nCompleting accurate and appropriate records in accordance with the professional standards policies and procedures of St Domingo House. This will include carrying out observations of residents within the home.\n\nMonitor risk and ensure the safety of residents, staff and relevant other visitors to the home.\n\nMaintain confidentiality, ensuring knowledge relating to GDPR is up to date.\n\nResponsibilities\n\nWorking hands on to ensure continuity of care, this involves working a regular shift pattern on rota, which consists of days, evenings, nights and weekends.\n\nTo complete the indication process to a good standard. Complete training and achieve an NVQ award within time frames and complete other relevant qualifications commensurate to the role.\n\nTo participate in the supervision and appraisal process and following recommendations as required.\n\nContributing to the planning process and being actively engaged in the support plan and risk assessment process.\n\nAttendance at staff meetings when required. This may also involve attending quarterly meetings and the dissemination of information to the residents and other staff members.\n\nTo ensure compliance with Health and Safety regulations and following recording and reporting procedures.\n\nTo record and report as required and share any discrepancies found within the home to the leadership team.\n\nIf an approved driver, to drive company vehicles with due care and attention, taking into account the need for safety and comfort of residents.\n\nEnhance Criminal Disclosure for the criminal records bureau is required for this role.\n\nExperience and Qualifications.\n\nMinimum 6 months’ prior experience in a care or support environment (residential, supported living, hospital, or community).\n\nLevel 2 Diploma (or equivalent) in Health and Social Care is preferred but not essential; full training and support will be provided to work towards this qualification.\n\nMandatory care training certificates (or willingness to complete): Fire Awareness, Health and Safety, Moving and Handling, First Aid, Infection Control, Safeguarding, Medication Awareness, Equality and Diversity and Food Hygiene.\n\nThe intended migrant meets these criteria and has verifiable experience and qualifications equivalent to the UK Level 2 Health and Social Care standard, evidenced by employer references and training certificates from their home country.\n\nSalary and Hours:\n\nSalary: £25,604.80 per annum (meets the Health and Care Worker visa minimum for SOC 6135).\n\nHours: 40 hours per week (full-time, shift-based including weekends and bank holidays as part of a rota).\n\nSkills, Experience, and Qualifications Required.\n\nThe role requires individuals who are:\n\nCompassionate, patient, and committed to person-centred care.\n\nAble to communicate effectively with vulnerable adults, families, and professionals.\n\nSkilled in managing challenging behaviour, emotional regulation, and supporting independence.\n\nReliable, flexible, and able to work both independently and as part of a multidisciplinary team.\n\nKnowledgeable about confidentiality, safeguarding, and infection control procedures.\n\nNot withstanding the detail within the job description, the post holder will undertake such duties as may be determined by the line manager from time to time, up to or at a level consistent with the main responsibilities of the post.\n\nThe job holder needs to be aware this post may evolve over time and consequently the job description may change in accordance with the needs of the Residents, home manager and the company
