Job Summary: The official job title is Lead Care Manager (LCM). The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support. QUALIFICATIONS: • Minimum of two years experience in direct training of developmentally disabled adults., • Minimum of AA degree or equivalent., • Must be able to type at least 35 words per minute and complete an assessment test prior to interview, • Demonstrates ability to provide staff training, supervision, and planning., • Communicate effectively with families, Regional Center, and Community Care Licensing, • Work professional with all stakeholders, staff, and management., • Strong Advocate for individuals with intellectual and developmental disabilities, • Communicate effectively, orally and in written form with strong organizational skills., • Good listener and follows instruction and not afraid to ask questions., • Ability to be flexible and embrace change., • Must be able to pass a pre-employment drug test., • Must have a good Health Report and tuberculosis clearance., • Must be able to obtain and required criminal records clearance, • First Aid and CPR required for the position., • Covid -19 Vaccinations and boosters recommended Values: Integrity, Innovation, Creativity, Advocacy, Quality, Sustainability, Education, Leadership, Kindness. 7:30 am - 4:00 pm
