Job Description Position Purpose: The Street Medicine Program Manager is responsible for the overall operational, administrative, and programmatic oversight of the Street Medicine program. This includes ensuring effective delivery of field-based healthcare services to individuals experiencing homelessness while maintaining compliance with grant requirements, organizational policies, and performance expectations. This role provides direct supervision of Street Medicine staff and is accountable for program performance, including productivity, patient engagement, care coordination, and reporting outcomes. The Program Manager works closely with providers and leadership to ensure services are delivered efficiently, safely, and in alignment with the Street Medicine model of care. This role requires a hands-on leadership approach, with active participation in field operations to support care team effectiveness. Principal Responsibilities • Provide direct supervision and leadership to Street Medicine staff, including hiring, onboarding, training, performance management, and disciplinary actions., • Oversee daily field operations to ensure staff adherence to assigned schedules, workflows, and site coverage., • Develop and maintain the monthly Street Medicine calendar in coordination with providers and leadership; ensure it serves as the operational source of truth and that all staff adhere to assigned locations and schedules., • Monitor and support program performance, including provider productivity targets and overall team efficiency., • Ensure all encounters are completed and closed by end of day, including accurate documentation, insurance verification, and correct Place of Service (POS) coding., • Ensure care teams conduct ongoing patient follow-up, care coordination, and referral completion, including housing, behavioral health, specialty care, and social services., • Oversee tracking and follow-up of high-risk patients, including those with chronic conditions, behavioral health needs, and HIV., • Ensure all HIV-positive patients are actively tracked, linked to care, retained in care, and supported toward viral suppression in alignment with EHE and HRSA requirements., • Ensure timely completion and submission of all grant reports (HRSA, EHE, HHIP) and required data tracking., • Monitor program data, including productivity, payer mix, patient engagement, and outcomes; implement performance improvement strategies as needed., • Facilitate daily huddles, multidisciplinary case conferences, and coordination meetings., • Maintain compliance with all JWCH policies, safety protocols, HIPAA requirements, and regulatory standards., • Maintain situational awareness during field operations and ensure staff safety., • Coordinate patient transportation services (e.g., Uber Health or approved alternatives) to support access to care., • Oversee readiness of field operations, including supplies, equipment, and mobile unit coordination (with appropriate delegation)., • Ensure staff are equipped with necessary medical supplies, equipment, and materials for daily field operations, in alignment with provider direction and program needs., • Collaborate with providers, ECM teams, behavioral health staff, and community partners to ensure coordinated service delivery., • Address and resolve operational issues, patient concerns, and workflow challenges in a timely manner., • Ensure staff are trained in workflows, documentation, and care coordination processes; support ongoing competency development., • Ensure Medical Assistants complete annual competency assessments, including clinical skills, EHR proficiency, and program-specific workflows; monitor performance and address gaps through training and corrective action as needed., • Monitor and enforce documentation timelines across EHR and reporting systems., • Prepare and submit regular operational reports and updates to leadership., • Support program planning, growth, and expansion initiatives., • Maintain relationships with health plans, community partners, and stakeholders., • Escalate operational issues through the established chain of command and ensure alignment with leadership., • Drive and operate program vehicles, including the medical mobile unit and team vehicles, to support field operations, including transporting staff, supplies, and patients, as appropriate and in accordance with organizational policy., • Support providers in daily field operations by facilitating efficient patient flow, assisting with coordination of visits, and ensuring readiness of the care team in the field., • Assist with real-time coordination of patient care activities, including locating patients, coordinating follow-up visits, and ensuring continuity of care in the field., • Ensure providers have the necessary operational support to deliver care effectively, including coordination of supplies, patient tracking, and site readiness., • Assist in resolving day-to-day operational barriers impacting providers, including scheduling conflicts, site issues, patient access challenges, and coordination with internal departments., • Support follow-up on urgent or priority patients, including those requiring immediate labs, referrals, or re-engagement., • Facilitate communication between providers and care team members to ensure alignment on patient care plans, follow-up, and service delivery., • Assist with coordination of same-day needs in the field, including transportation arrangements, patient access issues, and logistical challenges., • Bachelor’s Degree in Healthcare Administration, Public Health, Social Work, or related field required. Master’s Degree preferred., • Minimum of three (3) years of healthcare program management experience required. Street Medicine experience is required., • Demonstrated experience managing multidisciplinary teams and field-based operations., • Strong understanding of care coordination, healthcare operations, and program management., • Experience with grant-funded programs (HRSA, EHE, HHIP) and reporting requirements preferred., • Experience working with individuals experiencing homelessness and high-acuity populations., • Ability to manage program performance, including productivity, patient engagement, and quality outcomes., • Strong leadership, organizational, and problem-solving skills., • Ability to operate in fast-paced, high-demand environments and make sound decisions., • Experience with electronic health records (NextGen preferred) and reporting tools., • Strong communication and interpersonal skills., • Knowledge of HIPAA and patient confidentiality requirements., • Must be willing and able to actively support providers and care teams in the field on a daily basis, including real-time problem solving, coordination of patient care activities, and hands-on operational support during outreach. Preferred Qualifications • Master’s Degree in Healthcare Administration, Public Health, or related field., • Experience working within Street Medicine programs in Los Angeles County., • Familiarity with NextGen, Formsite, REDCap, and population health tools., • Experience managing HIV-focused programs, including Ryan White or EHE initiatives. This position operates in both field and administrative settings, including encampments, shelters, interim housing sites, and mobile medical units.
