Job Description Position Summary: The Registered Nurse – Women’s Health & Care Coordination plays a key role in delivering high-quality, patient-centered care within Wesley Community & Health Centers, a federally qualified health center (FQHC). This 1.0 FTE role includes coordination and oversight of Women’s Health programming, including the Well Woman HealthCheck Program and Title X services, as well as clinical activities such as Chronic Care Management (CCM) and direct patient care. The RN provides care under the supervision of licensed clinicians and in accordance with standing orders, clinical protocols, and regulatory requirements. Women’s Health Program Coordination and Oversight • Coordinate and support the day-to-day operations of Women’s Health programs, including Well Woman HealthCheck and Title X services., • Ensure program services are delivered in compliance with federal, state, and grant-specific requirements., • Serve as a clinical resource for staff providing women’s health services, including family planning and preventive care., • Assist with program reporting, quality improvement activities, and preparation for audits or site reviews., • Provide direct patient care under standing orders and clinician supervision, including but not limited to:, • Chronic disease management (e.g., diabetes, hypertension, hyperlipidemia)., • Sexually transmitted infection (STI) screening, education, treatment, and partner services as appropriate., • Birth control counseling, education, and follow-up within scope and protocol., • Preventive health screenings and patient education., • Participate in Chronic Care Management (CCM) activities, including patient outreach, care coordination, goal setting, and documentation., • Coordinate care across multidisciplinary teams, including providers, medical assistants, behavioral health staff, and case managers., • Support referrals, follow-up, and continuity of care for women’s health and chronic disease patients., • Provide culturally responsive, trauma-informed, and patient-centered education and counseling., • Adhere to all FQHC, HRSA, Title X, and organizational policies and procedures., • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR)., • Participate in quality improvement initiatives, chart reviews, and team meetings., • Adaptability - Adapts to changes in the work environment; manages competing demands; changes approach or method to best fit the situation; able to deal with frequent change, delays, or unexpected events., • Communications - Expresses ideas and thoughts verbally; expresses ideas and thoughts in written form; exhibits good listening and comprehension; keeps others adequately informed; selects and uses appropriate communication methods., • Continuous Learning - Assesses own strengths and weaknesses; seeks feedback to improve performance; pursues training and development opportunities; strives to continuously build knowledge and skills; shares expertise with others., • Customer Service - Manages difficult or emotional customer situations; responds promptly to customer needs; solicits customer feedback to improve service; responds to requests for service and assistance; meets commitments., • Problem Solving - Identifies and resolves problems in a timely manner; gathers and analyzes information skillfully; develops alternative solutions; works well in group problem solving situations; uses reason even when dealing with emotional topics. Education/Experience: • Graduation from an accredited school of nursing., • Minimum of two (2) years of clinical nursing experience, preferably in primary care, women’s health, or community health., • Experience working with underserved or vulnerable populations., • Knowledge of chronic disease management, preventive care, and reproductive health services., • Strong organizational, communication, and care coordination skills., • Experience in an FQHC or other safety-net setting., • Familiarity with Title X, Well Woman HealthCheck, or similar grant-funded programs. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Mathematical Ability: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills: • Microsoft Office Suite (Word, Excel, PowerPoint, Access, Publisher, Outlook, Microsoft Edge), • Current, unrestricted Registered Nurse (RN) license in the state of practice, • Fingerprint Clearance Card The noise level in the work environment is usually moderate.
