Job Description The Behavioral Health Technician (BHT) provides one-to-one behavioral management interventions to the child or adolescent with a serious emotional disturbance, in the assigned home, school or community setting, in support of the family or other responsible adults, in order to prevent more restrictive services or out-of-home placement. The BHT carries out specific behavioral management interventions identified in the treatment plan for the children or adolescent(s) assigned to their care. This role may involve the provision of 1:1 service to a child or adolescent. Services may also be provided to integrate or reintegrate the child into normalizing settings and activities. The BHT is responsible for providing behavioral health interventions to stabilize the child, assist the child in developing age-appropriate self-management skills, and to support, and empower parents, family members, teachers or other adults responsible for supervising the child in an educational or other community setting. The BHT works with the child, school staff, family and the entire treatment team by implementing the plan techniques in order to achieve the desired goals. THE FOLLOWING ARE THE ESSENTIAL FUNCTIONS OF THIS POSITION: Communication • Maintain contact with treatment team members (schools, families, wraparound staff, and other relevant parties)., • Clearly and effectively communicate pertinent information to responsible parties., • Advocate for the needs of the consumer to accomplish treatment objectives., • Communicates pertinent treatment information to the Clinical Supervisor., • Completes all progress notes within 72 hours of service to assure appropriate billing of services, • Completes all documentation in a timely fashion and in required formats, including, but not limited to progress notes, incident reports, and encounter forms., • Plans and organizes interventions that support in the social, emotional and educational development of the consumer, based upon the Treatment Plan., • Attends interagency meetings to ensure continuity of service., • Attends mandatory in-service trainings., • Participates in Performance Improvement Activities., • In accordance with the principles of best practice, provides oversight and direction to assigned children., • Assist the parent, teacher or other responsible adults in their efforts to provide direct supervision of the children., • Maintain coordination through interagency meetings., • Provides ongoing support to consumers and other staff as determined by the Written Order., • Creates an environment which fosters positive growth and development for all consumers., • Provides interventions to promote social growth in all consumers., • Directly implements components of the treatment plans developed in conjunction with the treatment team., • Represents agency at conferences, training and interagency meetings, as needed., • Ability to communicate and interact with all staff., • Ability to define problems, collect data, establish facts and draw valid conclusions., • Ability to effectively present information and respond to questions from varied groups, including the media., • Ability to accurately calculate numbers, such as in addition, subtraction and percentages., • Ability to give clear and concise oral and written instructions., • Excellent working knowledge of the Behavioral Health system. Job Description BAYADA Home Health Care is currently seeking a Part Time Admissions Coordinator to join our Hospice Triage Services team for Weekend Coverage. POSITION SUMMARY: The Admission Coordinator (AC) is responsible for receiving and coordinating hospice referrals from physicians, hospitals, facilities, and other community resources. The AC coordinates all hospice referrals and is the lead coordinator of all client referral/admissions activity. This role collaborates with different departments to obtain necessary billing and clinical preadmission information and build effective working relationships within the work group to benefit our clients and referral sources. The position works with and supports the families and clients directly at the beginning of their hospice experience, which may include explaining benefits, coordinating care, and sharing knowledge about the services provided. Confirm and obtain Do Not Resuscitate., • Coordinate hospice durable medical equipment/ comfort kits in place upon client arrival and with hospital staff upon discharge to ensure a smooth transition for the client when arriving home., • Confirm and audit client charts in HCHB, ensuring all necessary documents are signed, dated, and uploaded to HCHB., • Participate as a member of the marketing team by following up on pending referrals, developing and maintaining positive relationships with referral sources, and assisting in developing strategies and plans for site and company growth., • Provide the Billing Coordinator, or designee, with insurance and billing documentation, client location, and client level of care as needed for final confirmation prior to admission., • Maintain communication with pending clients, families, and referral sources and routinely follow the progress of any non-admissions., • Participate in the service office morning stand-up calls., • Ensure operations remain in accordance with BAYADA policies and procedures, and applicable local, state, and federal regulations., • Attend in-services and continuing education programs when appropriate and assigned.
