We are currently seeking a full-time Registered Nurse Case Manager to join our rapidly expanding team, serving clients throughout New York City. As a Case Manager, you will be responsible for developing and managing individualized care plans, ensuring that clients' evolving needs are met with precision and compassion. This role involves approximately 70% fieldwork and is designed for someone who excels with a smaller, detail-oriented caseload. You’ll have the opportunity to build meaningful relationships with your clients while playing a vital role in their care journey. This is a full-time position with a competitive salary ranging from $110,000 to $130,000, depending on your experience.
