Job Description The Vice President of Physical Health is responsible for overseeing the administration, coordination, and strategic direction of physical health services of the Integrated Health department of GSS. This is a key leadership position that will play a pivotal role in advancing our physical health services and ensuring their alignment with both organizational goals and clinical standards set by Medicaid Managed Care/A 29i. The Vice President is a member of the agency’s Agency Leadership Team. The Permanency Specialist position is a full-time role working directly with youth living in congregate care facilities or foster home settings who are in need of a permanent family to prevent them from aging out of care without permanency. The Permanency Specialist uses established, youth engagement and family recruitment methods to develop a trauma-informed plan to help youth achieve permanency by identifying adults in the life of the youth who are willing and able to answer the call and make an unconditional lifetime commitment to parenting. Operating out of Richmond County Criminal Court and community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in so doing, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC seeks a Housing Specialist for the borough’s Supervised Release Program (SRP). Reporting to the Community Engagement Coordinator, the Housing Specialist will leverage a deep and comprehensive knowledge of housing opportunities and challenges in the NYC area to provide in-house assistance to participants experiencing housing instability. Responsibilities will include assisting participants in obtaining both short-term and permanent housing opportunities. The Housing Specialist will work closely with the Clinical Director to build and maintain relationships with and have knowledge of local housing providers to complete housing referrals and help ensure case management staff are knowledgeable about housing providers and opportunities by facilitating in-house information sessions. Supervisors will assist in the overall functioning of the program with the goal of reducing the number of children who enter into the foster care system. Quality Assurance * Ensure Family Team Conferences (FTCs) are being held within a timely manner (within 4 weeks of FASP due date, as well as uploaded using proper coding. * Review and ensure quality of case notes, FASP reports, etc. * Ensure timely uploads of casework contacts, notes (within 5 business days), FASP reports, etc. All contacts must be added to the Preventive Organization Management Information System (PROMIS) by the 5th of the following month. * Review casework documents (legal documents, furniture requests, Investigative Consultation (IC) forms, etc.). * Collaborate with the Program Director to ensure program meets all city and state funding targets and benchmarks (for instance, safety assessments, case work contacts, family team conferences, preventive based funding targets, etc.). * Provide assistance to ensure that program is in compliance with all case planning and supervisory documentation. * Ensure confidentiality requirements are observed and practiced. * Attend internal and external trainings. * Assist in intake component of the program: Joint Home Visit (JHV), assessing Advocate Preventive Only (ADVPO) cases, etc. * Ensure monthly casework contacts are conducted. * Ensure case files are up to date. * Ensure quality casework practice, which includes safety and risk assessment of children and families and case progress. * Appropriately follow incident reporting procedures. Principal Responsibilities • Assign advocates to all residents upon arrival to NSP. >Facilitate no less than 5 therapeutic group meetings consistently, • Assist in the development of a comprehensive program schedule/activity plans w/site director., • Assist the Unit Manager in facilitation of weekly team meetings., • Coordinate all staff schedules two weeks in advance with final approval from site director., • Secure staff for all appointments, i.e. medical, court, probation, outside therapy, etc., • Meet with each shift bi-monthly for group supervision around strengths/challenges. Submit documentation of meetings within 48 hours to site director/NSP residential director., • Meet w/ NSP residential director bi-monthly; attend all meetings/trainings as assigned., • Ensure all shifts maintain a minimum of a 1 to 4. In the case the unit (on any and all shifts) is short staffed the group leader is required to fill in., • Support SCM trainings upon certification., • Collaborate with all services providers both internally/externally to meet needs of youth., • Support residents in working on treatment activities and petitioning for phase in a timely fashion., • Hold all shifts accountable (morning, evening, over-night Monday through Sunday) to follow the schedule and implement all activities as required., • Attend no less than 3 treatment planning conferences per resident per month (1stmtg, 3rd month meeting and final discharge meeting)., • Organize resident drop off and pick up, assign staff as needed and ensure all residents are back in the facility no later than 5pm on Sunday., • Attend all special events e.g., family day, St. John’s day, community service projects, trips, etc., • Additionally, beyond the aforementioned statements, perform any and all tasks as assigned by program leadership. Job Description Position Overview: The clinical supervisor organizes, administers, and oversees the clinical operation of the Specialized Treatment Programs utilizing evidence based best practice models and sound clinical judgment. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program in our licensed residential facilities located in Queens, Brooklyn, and Bronx as well as a Transitional Foster Care program in foster homes in the Bronx, Queens and Westchester. Job Description Program Summary The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program in residences located in the greater New York, as well as in Foster Homes in our Transitional Foster Care Program. The Training & Clinical Coaching Coordinator for the ACS Clinical Support Program collaborates with the Senior Director Clinical Support Program and Serving the community of State Island, the position will: Responsibilities: · Collaborate with the CPS Team and the Director to identify the training needs of the Safe Horizon and CPS partners · Provide Tech support and coordination · Training Development & Delivery · Provide needed coaching and consulting to SI CPS team · Utilize Conx, Promis , CMS and other data tracking systems needed to support the clinical program · Partner with ACS leadership and Clinical Partners to identify borough-specific training needs and priorities informed by case reviews, field trends, and staff feedback. · Design, develop, and deliver in-person and virtual training for CPS teams, Clinical Partners, and community providers. · Coordinate with ACS management to integrate relevant policies, treatment philosophies, and evidence-based practices into training curricula. · Adapt and deliver ACS-developed training materials when required to advance citywide policy or practice initiatives. · Maintain a regularly updated training calendar shared across boroughs to maximize attendance and engagement. · Ensure that all training materials are accessible, standardized, and stored in shared systems for easy reference. · Program Coordination & Evaluation · Track training participation, completion, and feedback; prepare monthly and quarterly reports for internal and ACS review. · Collaborate with the Research and Evaluation team to evaluate the effectiveness of training content and delivery. · Identify gaps in practice knowledge and propose new training modules or refreshers as needed. · Maintain ongoing communication with Clinical Partners to ensure cross-disciplinary learning (mental health, IPV, and substance use). · Serve as liaison between Safe Horizon’s Training and Learning Department and ACS Workforce Institute. · Coordinate guest speakers, facilitators, and cross-agency training collaborations. · Represent Safe Horizon at professional meetings, learning collaboratives, and citywide training events. · Will hold a race equity lens while developing training materials · Provide engaging, interactive training that incorporates the principles of adult learning · Partner with CPS, the Project Director, and the Safe Horizon Research & Evaluation team to evaluate the efficacy of and satisfaction with training · Will attend case conferences and other meetings as a way to assess the learning needs of the CPS and Safe Horizon Partners • Perform other related duties as assigned Qualifications: Education: Master’s degree in Social Work, Psychology, Public Health, Education, or a related field. Experience: Minimum five (3) years of experience designing or delivering training within human services, child welfare, or clinical settings. Strong understanding of trauma-informed care. Prior experience working with ACS or similar government agencies preferred.
