Job Description General Purpose: The Behavioral Health Counselor is working with a PROS program to conduct individual and group services. Personalized Recovery Oriented Services (PROS) is a comprehensive model that integrates rehabilitation, treatment, and support services for people with serious mental illness and possible other co-occurring disorders. Essential Functions and Key Responsibilities: • Coordination of Services & Plans:, • Develop, update, and ensure implementation of individualized treatment plans (e.g., CFAs, IPPs)., • Ensure plans include active treatment, activity schedules, and are regularly reviewed for relevance and accuracy., • Interdisciplinary Team Participation:, • Collaborate with nursing staff, DSPs, day program staff, clinical professionals (e.g., BIS), and family members., • Schedule and facilitate team meetings as required., • Advocacy:, • Represent individual preferences in areas including personal care, living environments, and access to services such as day programs or community inclusion., • Bridge communication gaps between residential and external programs., • Ensuring compliance & regulation:, • Maintain current knowledge of state and federal regulations (OPWDD)., • Ensure all service provisions align with agency policies and regulatory expectations., • Documentation & Billing Oversight:, • Maintain timely and accurate documentation, including progress notes and service plans., • Develop data collection systems and provide oversight for service documentation and billing., • Ensure billing submissions are accurate and timely., • Providing training & support:, • Train and support DSPs on the implementation of treatment and behavior support plans., • Ensure staff understand and consistently apply techniques and strategies., • Observation & Quality Assurance:, • Conduct regular observations (e.g., active treatment, mealtime, day program, and community inclusion)., • Use observation data to guide plan adjustments and support strategies., • Crisis Support:, • Respond to crises and collaborate with clinical staff in developing and monitoring crisis intervention plans., • Ensure support staff are trained and prepared to follow approved protocols., • Liaison with External Providers: Travel: • Travel between program sites (e.g., Brooklyn, Washington Heights, and Riverdale) is required., • Additional Requirements:, • Knowledge of OPWDD regulations and person-centered planning., • Strong written, verbal, and organizational skills., • Ability to respond professionally to crisis situations and exercise sound judgment., • Capacity to work both independently and as part of a multidisciplinary team., • Familiarity with data systems for documentation and billing oversight., • Physical Requirements:, • May assist with wheelchair transfers or mobility support.
