You will triage the order based on date and time needed, select and carefully pack life-saving blood products with the correct temperature regulating product to ensure safe transport. Job Description The Housing Locator, SSVF (Supported Services for Veteran Families) reports to the Team Leader, SSVF and is responsible for the development of affordable housing opportunities for Thresholds members by building relationships with landlords and identifying suitable rental apartment units for members. Housing Locator is responsible for the management of Thresholds residential properties and/or subsidies in accordance with all current funding requirements, Fair Housing Laws, and Thresholds policies and procedures. ESSENTIAL DUTIES & RESPONSIBILITIES: • Manages all Thresholds Supported Services for Veteran Families residential properties, subsidized and non‐subsidized, in accordance with all current funding requirements, such as but not limited to Housing & Urban Development (HUD), Veteran Affairs Supported Housing (VASH) or equivalent project-based voucher program, current release of the HUD 4350.3 Occupancy Handbook, HUD Continuum of Care Programs including Long Term Rental Assistance Program and HUD Leasing Program, Federal, State & City Fair Housing Laws and Thresholds policy and procedures., • Conducts and completes applicant interviews, oversees move‐ins, move outs, processes annual and interim certifications, verifications, paperwork, and data entry required in completing each process., • Maintains required information in Thresholds Homeless Management Information System (HMIS) data system., • Maintains veteran files in accordance with funding requirements and SSVF program policy and procedures., • Collaborates with HUD/VASH team in maintaining the housing waitlist., • Completes all necessary reports as determined by the SSVF program office and/or other funding sources as requested., • Obtain HMIS Certification, • Expedite housing placements with the appropriate level of services and Identify community resources & initial housing needs for Veterans., • Conduct Assessments for ALL HUD/VASH referrals., • Present weekly case consultations to VA HUD/VASH teams for clinical case management & referrals for SMI veterans. Job DescriptionPosition Summary: Casa Central’s Early Learning Academy (ELA) provides high quality, comprehensive early childhood education services for young children, ages birth to five, and their families. A key member of the program’s leadership team, the Disabilities & Wellness Promotion Coordinator supports the Early Learning Academy in providing services, advocacy, and meaningful inclusion for children with diagnosed disabilities, suspected disabilities, challenging behaviors, and other needs, and their families. The Disabilities & Wellness Promotion Coordinator provides coaching and training for teaching teams and families, supporting staff as they serve children and families with diverse learning needs. Job DescriptionPosition Summary: Under the direction of a Team Supervisor, this position is responsible for providing comprehensive home-based case management services to assigned families with the goal of promoting safety, wellbeing and stability, in accordance with the Illinois Department of Children and Family Services (IDCFS) requirements and Intact Family Services and Family First Prevention Services Act. Promptly responds to new IFS referrals by participating in the supervisory hand-off and transitional visit within the specified timeframe.
