Job Description Position Summary The RN provides care to clients within ADHS. The RN reports to and works under the supervision of the ADHS Center Director. The nurse renders all care as ordered by the client’s medical provider, acts as a member of the multi-disciplinary team and liaison between healthcare professionals, completes nursing assessments and reassessments, maintains records to ensure agency and regulatory compliance, reports any changes in orders and/or client condition to the supervising RN and client’s provider, and provides health education to staff, clients, and their families/caregivers. Essential Functions: % of Time Care Delivery • Performs and documents initial assessment and reassessments, • Provides nursing care to clients, including but is not limited to, administering medications, personal care, wound care, management of tracheostomy, feeding tube, or urinary catheter, • Monitors the health condition of clients, including but is not limited to, performing assessment, obtaining vital signs, and reporting changes in condition as needed, • Adheres to the nursing care plan as implemented by the RN in collaboration with the client, client’s family/caregiver, medical provider, and external case manager, • Maintains a record of all medication and treatments administered and care rendered as ordered by client’s medical provider, • Coordinates care and maintains open communication between the client, client’s medical providers, multi-disciplinary team, and client’s family or responsible party, • Completes Incident and Infection documentation when necessary, • Assumes the leadership role in all health emergency situations, calling emergency medical services when necessary, • Maintains a safe environment and immediately reports any hazards to supervising RN and/or Center Director, • Provides health education to staff, clients, and their families, • Conducts in-service trainings for staff, • Provides TB tests to staff annually per regulatory requirements Continuous Improvement and Quality • Ensures compliance with all AllThrive 365 policies, procedures, and practices regarding the contract(s) and licensure requirements; and applies knowledge to the work in accordance with quality standards, • Always performs in a professional manner as directed by the Nursing Scope and Standards of Practice and Code of Ethics, • Attends and actively participates in initial and on-going training and development programs and opportunities and demonstrates learned concepts on the job, • Engages in continuous improvement efforts, as assigned, and strives to continuously improve the quality of care and self, • Communicates daily and as necessary with colleagues, supervisor, and leadership 15% Miscellaneous • Travels between program sites as necessary, • Perform procedures, such as, outer cannula tracheostomy exchange, and recertification visits as needed Requirements Education/Experience/Credentials/Knowledge/Understanding • 18 years of age or older, • Have and maintain AZ Registered Nurse License, • Have and maintain Fingerprint Clearance Card, • Have and maintain CPR certification, • Have and maintain Food Handlers Card, • Have and maintain AZ Driver’s License and acceptable MVR, • Have and maintain reliable transportation and current vehicle insurance, • Meet all state and federal immunization and communicable disease clearance requirements, • This position is classified as Safety Sensitive Skills • Sound clinical judgment and decision-making skills, • Dexterity and physical ability to provide necessary care, • Excellent verbal, written, and listening communication skills to include communicating clearly, effectively, tactfully, and patiently to a diverse population, • Excellent interpersonal skills and demeanor with the ability to build rapport and garner trust with others, • Strong organizational, planning and time management skills, • Proficient in the use of a PC; in a Windows environment; in the use of the Internet; and in the use of MS Office Applications such as Outlook, Word and Excel Abilities • Able to comprehend, retain, and follow regulations and procedures relative to the provision of ADHS, • Able to complete documentation in accurate and timely matter, • Able to cope with a high-stress environment and work with various populations, • Able to work with challenging behaviors (e.g., verbal and physical aggression, wandering, etc.) expressed by clients, • Ability to solve problems by applying analytical skills to include:, • collecting and reporting information, • Identifying trends and exceptions, • investigating to define problems more accurately, • sorting information in order of importance, • identifying relationships and linkages between components, • identifying variable potential causes and effects, • soliciting guidance to define criteria and assign values of importance and urgency, • escalating issues of an exceptional nature, • Willingness and ability to embody AllThrive 365’s core values:, • COMPASSION, • GROWTH, • RESILIENCE, • OPENNESS Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. • Must be able to lift 25 pounds, • Requires bending, stooping, lifting, and standing for long periods of time, • Requires working in various environments and temperatures, • Requires driving vehicle and traveling to various program sites during daytime and evening hours, • Must not have any restrictions for physical work for which reasonable accommodation cannot be made
