Job Description Caregiver / Homemaker / Personal Care Assistant The Caregiver/Homemaker provides non-medical support to clients in their homes. Responsibilities include assistance with daily living activities, light housekeeping, meal preparation, transportation, medication reminders, and personal care support to help clients maintain safety, comfort, and independence. The caregiver also maintains clear communication and collaboration with physicians, healthcare providers, and care teams to ensure continuity of care and proper reporting of client needs or changes in condition. Meal Preparation (Homemaker Services) Assist clients with preparing and serving meals according to dietary needs. Responsibilities include: • Prepare and serve breakfast, lunch, dinner, and snacks, • Assist with feeding if necessary, • Follow dietary restrictions or physician-recommended diets Responsibilities include: • Laundry and ironing, • Changing bed linens, • Cleaning out refrigerator, • Emptying trash, • General tidying of living spaces Responsibilities include: • Provide morning, afternoon, and evening medication reminders, • Ensure medication organizer/box is accessible and used appropriately, • Monitor for missed doses or potential concerns Responsibilities include: • Transport clients to medical appointments, • Assist with errands such as grocery shopping or pharmacy visits, • Escort clients safely to and from vehicles or buildings Responsibilities may include: • Verbal cueing, • Standby assistance, • Stand/pivot transfers, • Sliding board transfers, • Hoyer lift transfers (if trained), • Walker or cane, • Wheelchair, • Scooter, • Brace, • Hoyer lift Personal Care Assistance Assist with daily hygiene and grooming while maintaining dignity and privacy. Bathing Assistance • Shower assistance, • Tub bathing, • Bed bathing, • Shampooing hair, • Skin care, • Dressing assistance, • Oral care, • Shaving, • Nail care Assist clients with toileting and incontinence care. Responsibilities include: • Bathroom assistance, • Bedpan assistance, • Bedside commode use, • Urinal assistance, • Changing incontinence products (Depends), • Bed pad changes Maintain consistent communication with the care team to support the client’s health and well-being. Responsibilities include: • Work closely in communication and collaboration with physicians, nurses, case managers, and other healthcare professionals, • Report changes in the client’s condition, behavior, or mobility to the appropriate healthcare provider or supervisor, • Assist with relaying important information from medical appointments to the care team or family when appropriate, • Follow care plans established by healthcare providers Maintain accurate and timely records of care provided. Responsibilities include: • Document services provided and client observations, • Record notes regarding client condition or changes, • Ensure proper signatures from clients and representatives, • Maintain service dates and documentation according to agency policiesCompany Description"Quality of Life" is a lifestyle.
