As a Home Dialysis Nurse (HHD/PD), you will deliver high-impact, one-on-one clinical care and education directly in patients’ homes, empowering them to safely and confidently manage home hemodialysis and peritoneal dialysis. You will serve as a trusted clinical expert for patients, caregivers, and providers, ensuring quality outcomes, regulatory compliance, and continuity of care across the Broward County territory. About the Role This field-based role blends advanced clinical nursing, patient education, and independent practice. You will assess patient readiness, deliver competency-based training, monitor outcomes, and collaborate closely with nephrologists and interdisciplinary teams. While working autonomously in the field, you’ll remain closely connected to clinical leadership and quality teams. Strong clinical judgment, regulatory knowledge, and comfort working in patient home environments are essential. Key Responsibilities • Conduct patient assessments for HHD and PD, including clinical stability and home environment readiness, • Provide in-home training and ongoing clinical support for home dialysis therapies, • Deliver competency-based education on equipment use, infection prevention, troubleshooting, and emergency procedures, • Validate patient and caregiver competency for safe, independent home dialysis, • Collaborate with nephrologists and care teams to develop and manage individualized Plans of Care, • Monitor outcomes, adherence, and clinical indicators; identify risks and intervene appropriately, • Ensure compliance with CMS ESRD Conditions for Coverage, CDC, OSHA, and organizational policies, • Maintain accurate, timely, and compliant clinical documentation, • Participate in QAPI activities, survey readiness, audits, and corrective actions, • Support patient communication, care coordination, and dialysis continuity during emergency eventsRequired Qualifications, • Active, unrestricted Registered Nurse (RN) license in Florida, • Minimum of 1 year of home dialysis experience (HHD and/or PD), • Strong knowledge of CMS ESRD Conditions for Coverage, • Excellent clinical assessment, documentation, and patient education skills, • Valid Florida Driver’s License and ability to travel to patient homes, • Current BLS certificationPreferred Qualifications, • Experience participating in QAPI or survey readiness activities, • Prior experience training or mentoring nursing staffComprehensive Benefits Package, • Medical, Dental, and Vision Insurance, • Paid Time Off (PTO), • Paid Holidays, • Competitive salary: $90,000–$106,000, • Quarterly bonus potential up to $4,000, • Company vehicle provided
