Job Description Description of the Role: We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a full-time basis. In this role, you will be responsible for telephonic care management and support to patients in Allentown, PA and the surrounding counties. Responsibilities: • Reviewing patient assessments and developing individualized care plans with the care team, • Ensuring all skilled nursing care, including wound care, medication management, and IV therapy is provided as ordered, • Coordinating and communicating with healthcare team members to ensure comprehensive and effective care is provided, • Monitoring patient progress and adjusting care plans as needed, • Educating patients and their families on proper care techniques and self-management, • Valid Registered Nurse (RN) license in the state of Pennsylvania, • Minimum of 1 year of experience in home health or similar setting, • Minimum of 1 year of experience in a case management role, • Excellent clinical and critical thinking skills, • Strong communication and interpersonal skills, • Ability to work independently and manage time effectively, • Competitive compensation, • Flexible schedule, • Opportunity to make a meaningful impact on patients' lives Interim Home HealthCare is a leading provider of home health services, with a focus on delivering exceptional care and support to individuals in their own homes.
