Job Description Job title Case Manager Reports to Case Manager Supervisor Position Summary The Lodge (The Mental Health Recovery Lodge of Northampton County) is a supportive housing and recovery program for adults with mental illness in Northampton County. The Lodge provides affordable housing, and a supportive environment designed to promote recovery, stability and the inherent ability to succeed. The emphasis of the Lodge is not on managing symptoms, but on increasing the quality of life as defined by the individual. The case manager will determine resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. Guiding Principles of The Lodge • Celebrate Greatness in Self and Others, • Actively Implement the Mental Health Recovery Model, • Support the Pursuit of Happiness and a Meaningful Life for All Members and Staff - Aim to Thrive!, • Maintain a Daily Practice of Mindfulness and Self-Awareness, • Take Full Ownership for How We Feel, Think and Act, • Provide a Holistic Program (Body, Mind & Spirit), • Encourage Hope, • Celebrate Diversity, • Honor Respectful and Open Communication, • Value Creativity, Flexibility and an Entrepreneurial Spirit, • Apply Green Practices, • Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective., • Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant., • Provide community-based services to program participant focusing on education, connection with resources, community interaction and achievement of recovery plan goals., • Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities., • Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources., • Other duties as assigned Essential Duties Addendum • Approach residents/consumers with unconditional positive regard, • Adhere to program schedules, arrive for all shifts, meetings and trainings promptly, • Facilitate members meeting & work collaboratively with members/residents to create weekly programming schedule, • Ensure safety of consumers/residents by conducting safety checks, monthly apartment checks and evacuation/safety drills, • Be acquainted with all members and locations of their apartments, • Operate all areas of Café the Lodge including opening procedures, Point of Sale, Barista duties, Cooking duties & closing duties, • Provide crisis intervention as needed, • Assist with Butler Street Apartments as needed, • On-Call Stipend-$75 per rotation Qualifications • Bachelor’s Degree in Human Services or related field., • Intermediate computer skills, • One year Case Management experience preferred, • Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required., • Valid driver’s license Working conditions Hazard and Atmospheric Conditions • Exposure to Fumes, • Exposure to Dust, • Exposure to Extreme Heat, • Exposure to Extreme Cold, • Wet and/or Humid, • Exposure to Loud Noise, • Exposure to Confined Places, • Mists or Gases, • Exposure to Mechanical Hazards, • Exposure to Chemical Hazards, • Exposure to Electrical Hazards Working out in community within the service delivery area of the program. Required to drive program participants.
