Provide between-visit patient contact with patients, providing necessary medical reminders, instructions about medications/adherence., • Develop a strong collaborative relationship with the HIV medical community to enhance patient access to services including specialists., • Comply with the Equitas Health Health Care Corporate Compliance Standards of Conduct and related policies and procedures., • Participate in preparing statistical and other reports as needed., • Play key role on multidisciplinary team in providing a medical home for patients. Job Description Nurse Practitioner (Licensed NP) for Mobile Wound Healing LLC (Cincinnati, OH) as a member of the medical team that specializes in wound care. Mobile Wound Healing LLC delivers wound care services at patients' residences inclusive of their homes, Assisted Living Facilities, and Independent Living Facilities. This role involves providing care for patients by assessing and monitoring wounds and making clinical decisions while creating/maintaining a plan that results in successful wound care management.
