Job Description Purpose of the Role: The purpose of the Social Care Network is to connect Medicaid recipients to resources that can be found within the Hudson Valley community. These resources can be local, state, or federally funded programs. Essential Functions of the Role: A Social Care Network Screener helps individuals access community resources and services to address their health-related social needs (HRSN). Through the specific questions of the SCN tool, screeners are able to help our community members identify what their needs are and help them address those needs by connecting them to resources they might not have known existed. This role involves screening to ensure individuals receive the support they need, whether it's access to housing, transportation, nutrition, or other essential services. Key Responsibilities of a Social Care Network Navigator: • Screening: Evaluating individuals' needs and determining eligibility for various community resources and programs., • Outreach and Education: Reaching out to individuals to inform them about available services, particularly those who may be underserved or unaware of their options., • Advocacy: Supporting individuals in advocating for their needs and rights within the system, ensuring they receive equitable access to care., • Documentation and Reporting: Maintaining accurate records of all activities, referrals, and outcomes to track progress and improve services., • Strong interpersonal and communication skills are essential for building rapport and trust with individuals., • Knowledge of community resources, social service programs, and health care systems is crucial for effective navigation., • Empathy, patience, and a commitment to serving diverse populations are valuable attributes or this role., • Experience in case management, social services, or a related field is often preferred. Internal: Supervisor of Intake and Outreach, Program Administrator, Asst. Program Director, Client Care Supervisor, and Client Care Coordinators. External: Social Care Network providers: Dept. of Social Services, Mental Health Providers, Primary Care Physicians, Probation Officers, Legal Services, District Attorney’s Office, etc. Work Schedule: 9 am – 5 pm Physical Environment: • Traditional office environment., • Must be able to interact with people having mental health issues, periods of instability (i.e. lack of shelter, food, clothing or support), recently released from an institution (i.e. hospital, prison), limited comprehension and/or ability to manage through complex scenarios.
