Job Description • Performs the daily Exam room preparation and inventory management, • Correctly prepares exam room, • Sets up appropriate trays and equipment for testing to be done during the visit, • Cleans/disinfects room according to cleaning standards, • Stocks exam rooms per clinic standards, • Secures equipment and supplies at the end of the clinic day, • Assures that sufficient supplies are available, • Forwards supply order to site manager/administrator per clinic procedure, • Checks for expired supplies and medications on a monthly basis and removes the expired supplies and gives to the site manager/administrator, • Inventories patient assistant medications daily, as per clinic procedure, • Prepares Patient & Rooming Documentation per Clinic Standard, • Performs chart audits in preparation for appointments, • Verifies patient identity, • Obtains and documents patient history, • Obtains and documents patients chief complaint/reason for visit, • Obtains and documents vital signs, • Height, weight, BMI, BP, RR, Pulse, Oxygen saturation, • Length, weight, head circumference, BMI, BP, RR, Pulse, Oxygen saturation, • Rechecks vital signs if first set was out of normal range, • Completes and documents screenings appropriately: tobacco, alcohol, recreational drugs, fall risk, PHQ-9, • Reviews allergies and documents accordingly, • Reviews chart alerts and recommends appropriate referrals, • Initiates patient education according to the individual needs of the patient and gaps in care, • Elicits and incorporates patient feedback regarding the plan of care and notifies the nurse or provider, • Notifies provider/nurse that the patient is ready to be seen, • Records in the electronic health record the disposition of missed appointments, no show appointments, patients that left without being seen and then initiates tracking, • Makes phone calls and mails letters according to provider instruction per clinical policy, • Provider Assistance and Minor Procedures, • Chaperones provider as needed and during sensitive exams, • Assists provider with physical exams as needed, including pelvic exams, pap smears, • Prepares equipment and assists provider with specimen sampling, point of care testing and procedures, • Assures that consent is given and documented appropriately, • Assures that time outs are performed and documented appropriately, • Administers prescribed medications and injections per policy, • Performs dressing changes, ear lavage and eye irrigations according to policy, • Assures cleaning and sterilization procedures are adhered to according to policy, • Checklist / Log Completion and Labs, Testing and Equipment, • Complete the daily logs of the following, • Eye wash stations: bump and flush test, • Oxygen tanks, • Refrigerator temperature log, • Equipment log, • b. Point of Care Testing, • Glucose testing and controls, • Hemocue testing and controls, • Hemoglobin testing and controls, • Urinalysis testing and controls, • Urine pregnancy test testing and controls, • Fecal occult blood testing and controls, • Influenza A &B testing and controls, • Strep A testing and controls, • c. Specimen Collection, • Nasopharyngeal swabs, • Throat culture, • d. Testing Set-up and Completion, • EKG, • Peak Flow Meter, • Visual Acuity, • Audiometry, • Infection Control/Prevention, • a. Follow PHC Transmission-based precautions, • b. Follows standard precautions with all patients, • c. Demonstrates WHO guidelines for hand hygiene with soap/water and hand sanitizer, • d. Demonstrates cleaning, disinfection, sterilization of instruments and high-level disinfection of equipment, • e. Inspects multi-dose vials/bottles for contamination and discards appropriately if it appears contaminated, • f. Indicates opening date on a multi-dose vial/bottle and discards at the end of 28 days, • Quality Measures and Continuity of Care, • Instructs/escorts patients to the next level of care for that day (x-ray, lab, pharmacy), • Schedules appointments for continuity of care and to close care gaps, • Manages tasks in NextGen and Scans documents as needed, • Reviews and use Previsit planning sheets daily to prepare for appointments, • Discuss preventative services with the patients and documents any barriers, • Work with FOA/Scheduling if unable to perform scheduling needs, • Plan, organize and participate in quality projects appropriate to department, • Review monthly quality reports, encourage and lead team to meeting measure goals
