This role incorporates the tenets of Mount Sinai’s Relationship Centered Care model, focusing on collaboration, patient experience, and quality outcomes. Key Responsibilities: • Deliver patient-centered care utilizing the nursing process and critical thinking., • Collaborate with interdisciplinary healthcare teams for plan-of-care development and evaluation. Educate patients and caregivers on health promotion, transition of care, and disease prevention.
