Job Description We are seeking a compassionate and skilled Nurse Practitioner (NP) to join our Home-Based Palliative Care Program, providing high-quality, patient-centered care to individuals with serious, chronic, or life-limiting illnesses. This unique role will be embedded within community medical practices and involve regular home visits to support patients in the comfort of their homes. Salary for this position is $100k-$120k annually and is commensurate with education and applicable experience. Hospice and Palliative Nurse Practitioner The Hospice and Palliative Nurse Practitioner assists the Medical Director with oversight and execution of the medical component of hospice and palliative services. Provides clinical support and education to hospice and palliative multi-disciplinary team. Collaborates with internal and external providers and referral sources to expand access to services. Serves on agency Palliative Care program development committee. The Hospice and Palliative Nurse Practitioner will work within an interdisciplinary team to provide consultation to patients with serious illness who are seeking assistance with advanced symptom management, clarifying goals of care, addressing end-of-life issues and care, and transition planning. Projected to spend 80 percent of the time with hospice patients and 20 percent of time working on the creation of a home-based palliative Care Program and outreach to physicians to increase hospice referrals. Responsibilities: Palliative • Provide in-depth palliative care assessments and ongoing management for patients referred to the program., • Conduct home visits to assess clinical status, manage symptoms, and address patient and caregiver needs., • Serve as a clinical resource embedded within community medical practices to support early identification and management of patients in need of palliative care., • Develop and implement individualized care plans in collaboration with physicians, social workers, and other team members., • Offer emotional and psychosocial support to patients and families, including goals-of-care discussions and advance care planning., • Coordinate care transitions and collaborate with home health, hospice, hospital teams, and other community resources., • Participate in interdisciplinary team meetings and case conferences., • Maintain accurate, timely clinical documentation in accordance with organizational standards and regulatory requirements., • Diagnose and treat acute, episodic, or chronic illnesses, • Prescribe all necessary medications and treatments, • Perform comprehensive physical examinations of patients, • Develop and implement patient management policies and procedures, • Facilitate referrals to other healthcare professionals and medical facilities, • Communicate with collaborating physicians or specialists regarding patient care Hospice • The Hospice Nurse Practitioner provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosis of medical conditions, and management of health problems by directing and developing the plan of care, coordinating, and collaborating with patients, families, and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments, and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team., • Certifies and re-certifies patient eligibility for the Hospice Program in conjunction with the attending physician on a timely basis., • Provides clinical support to hospice Multi-Disciplinary team., • Consult with primary physicians or specialists as needed regarding the patient's medical condition. Assume responsibility for the general medical needs of Hospice patients to the extent that the attending physician does not meet the needs, serving as attending physician as needed and when requested by the patient or their representative. Work with attending physicians to encourage the fulfillment of their responsibilities., • Serves as a liaison between the attending physician and the Hospice Team by obtaining/sharing information about the patient's plan of care on an ongoing basis., • Evaluate the patient's medical status in the home, outpatient, and inpatient settings based on a visit schedule., • Works within the hospice provider team to ensure adequate delivery of care 24/7. Serves on the clinical leadership team., • Attends Patient Care Conferences as requested., • Provides ongoing community and professional education as it relates to the medical community. Routinely networks with hospital staff to provide educational seminars on hospice., • Assists with the design and development of a home-based palliative care program., • Supervised the care of palliative care patients in the home-based program for the agency. Serves in a consultative manner to the patient’s attending physician. Qualifications: • Master's degree and graduate of a New York State approved Nurse Practitioner program. Currently licensed and certified as a Nurse Practitioner by a national certifying organization acceptable to New York State in a related practice specialty area., • Completion of the pharmacotherapeutic requirement enabling independent prescriptive privileges., • Must have a National Provider Identifier (NPI) and Federal Drug Enforcement Administration (DEA) Registration Certificate in order to prescribe controlled substances., • Previous experience in nursing or other medical fields, • Ability to build rapport with patients, • Ability to thrive in a fast-paced environment, • Excellent written and verbal communication skills, • Strong leadership qualities. Five years of experience including pain and symptom management experience., • Proof of completed COVID-19 vaccine Work Environment & Schedule • Hybrid community-based role with time split between medical practices, patient homes, and occasional remote work., • Flexible scheduling to meet the needs of patients and practices., • Participation in weekend, on-call coverage, and after-hours may be required.
