Summary The Clinical Pharmacist plays a key role in advancing population health initiatives and optimizing performance across value-based care (VBC) programs. This role will support eight community health centers (CHCs) and contribute to clinical programming, quality improvement, and risk adjustment initiatives within Accountable Care Organizations (ACO), Medicare Advantage, and other VBC contracts. This role has a deep understanding of cost and quality parameters, payer expectations, and CMS-HCC risk adjustment, as well as experience analyzing data to drive meaningful clinical improvements. The Clinical Pharmacist plays a critical role in medication optimization, provider education, population health strategy, and quality performance in a value-based care environment. The clinical pharmacist thrives in a data-rich setting and is passionate about improving outcomes through innovative, team-based care. What You’ll Do Strategic Engagement & Leadership Support: Attend and contribute to: • Board meetings, • Quarterly Payer Joint Operations Committee (JOC) meetings, • Quarterly Performance Review Meetings with eight CHCs, • Provide data-driven informed and clinical recommendations as needed., • Conduct ad-hoc site visits to CHCs to assess performance and offer on-site support., • Serve as a subject matter expert on ACO, Medicare Advantage, and value-based payer contracts, including: Cost and quality performance metrics and Payment methodologies., • Identify, educate, and mitigate medication-related problems • Adherence, • Side effects, • Interactions, • Affordability, • Appropriateness • Collaborate with care teams to implement evidence-based pharmacotherapy interventions, • For CHCs without a clinical pharmacist, serve as a medication expert for primary care providers, care managers, and behavioral health teams, • Assist in the development and refinement of clinical workflows related to chronic disease management and preventive care, including but not limited to diabetes, hypertension, and depression., • Develop and help deliver a comprehensive training and ongoing education program for CHC quality and clinical teams., • Ensure clinical teams are up-to-date on: • HEDIS, STAR, and other quality measures, especially around medication adherence and therapy intensification, • Documentation and coding best practices, • Preventative, chronic disease, and wellness care models • Analyze prescribing trends and medication-related quality measures to identify gaps and opportunities., • Partner with data and analytics teams to design and refine reports related to pharmacy outcomes, • Provide education on formulary optimization, prior authorization workflows, and emerging clinical evidence, • Stay abreast of federal and state policy developments impacting 340b programs, FQHCs, Medicaid, and uninsured populations, • Provide timely education and strategic guidance to internal teams, partners, and the network on the clinical and operational implications of proposed or enacted legislation, • Partner with physician leaders and population health teams to design scalable and repeatable workflows that drive: • Improved quality performance metrics, • Reduced emergency room and inpatient utilization, • Enhanced patient engagement and health outcomes What We’re Looking For • Doctor of Pharmacy (PharmD) required., • Minimum of 3–5 years’ experience in a clinical pharmacist role within a VBC, ACO, or health plan environment., • Work with honesty and integrity, • Displays a positive and respectful attitude, • Represent the organization in a responsible and professional manner, especially when serving members and communicating with peers, • Provide excellent customer service to members, • Perform job duties to a reasonable, acceptable standard, • Maintain excellent communication with the team and members Working and Physical Requirements • This position may require periodic travel to company events or professional meetings in the state of Colorado, or for meetings of related professional organizations, • Prolonged periods of sitting at a desk and working on a computer, • Must be able to access and navigate each department at the organization’s facilities, • May include remote and in-person responsibilities depending on organizational needs, • Able to work after hours when needed, • Predominately a remote position, with flexibility to travel to the office when required, • All staff are required to participate in the
