A, • copy of the summaries will be submitted to the Director of Nursing, • Treat clients, families, and staff with courtesy and respect Requirements, • Graduated from an accredited physical therapy program by the American Physical, • Therapy Association, • Valid licensure in state where care is given, • Comply with all state mandated Physical Therapy Practice Act and other legal mandates, • Adhere to HIPAA and maintain client confidentialityRequirements:, • Graduated with a masters or higher degree in physical therapy from an accredited physical therapy program, • Passed a National Physical Therapy Examination (NPTE) and/or state examination required by the state, • Wisconsin PT license required, • Two (2) years of appropriate experience as a physical therapist. Community/home health is preferred, • Adhere to HIPAA and maintain client confidentiality, • Certified CPR and negative TB skin test or chest x-ray, • Licensed driver with valid automobile insurance, • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull., • Varied assistance is required depending on the mobility of the client.
