Job Description Title: Nurse Manager Department: Primary Care Services Reports To: Senior Director of Health Services Shift: FT/ 8:00AM - 5:00PM CST / M-F Location: All Health Centers: 5501 S Halsted, 4009 W Broadway Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community. Heartland Alliance Health is seeking a dynamic Nurse Manager to oversee our dedicated and skilled team that provides health care to individuals experiencing homelessness. In this role, you will provide leadership, mentorship, and operational management to ensure the delivery of high-quality, trauma-informed care to individuals experiencing homelessness. The Nursing Manager is responsible for managing the functional and clinical oversight of Nursing activities including participant care, group education, and infection control activities. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below. Essential Duties and Responsibilities: • Manages nurses through the ongoing development and implementation of routine education, training and clinical oversight., • Leads huddles and engagement efforts., • Collaborates with Management of Health Centers and Shelter-Based Care to periodically review and revise the medication and supply formulary. Completes routine inventory and management of medications and supplies., • Collaborates with the Senior Director of Health Services, Lead Providers to periodically review and evaluate nursing procedures, staff workflow, and equipment requirements to optimize operational effectiveness., • Assures optimal quality and safety of patient care through monthly review of environment of care, quality outcome and performance measures, and compliance with all regulatory agencies, including the Illinois Department of Public Health and the Joint Commission., • Carries out duties of their assigned staff when staff shortages persist., • Develop workflow, operating procedures, management reports and instructional/training materials required for the evaluation and adoption of new processes and technologies., • All aspects that are included in the management of assigned staff such as paperwork for hiring, disciplining, compensation, payroll and timesheet approvals, performance management, corrective action plans, and etc., • Assist with special projects as needed., • Ability to work out of multiple HAH health centers Shelter-Based Care locations and travel to account visits., • Bachelor’s degree in nursing required equivalent with at least three to five years relevant experience as a registered nurse., • Master’s degree in nursing and/or public health and public health work experience highly desired. Other Qualifications: Demonstrated high degree of computer proficiency, including demonstrate skills in MS Office such as Word, Outlook, Excel, and Electronic Health Record keeping software required., • Language Skills: Ability to write reports, correspondence, and instructions. Ability to effectively present information and respond to questions from staff, managers, participants, vendors, and the general public., • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs., • Reasoning Ability: Ability to apply common-sense understanding to carry out instructions furnished in written, oral, or diagram form.
