As a Rural Health Transformation Program (RHTP) Regional Grant Coordinator, you will help turn Indiana’s rural health vision into action across your assigned region. This role supports the Make Rural Indiana Healthy Again (MRIHA) Regional Grants, a major multi-year investment to improve rural healthcare access, prevention, and system resilience. As a remote, field-based contractor living in your assigned region, you will serve as a key link between IDOH, regional coalitions, regional leadership, and statewide technical assistance partners. *See the region list below. In this role, you will: • Manage regional grant and contract activities for organizations receiving RHTP funding, • Monitor compliance with state and federal requirements, budgets, deliverables, and timelines, • Identify risks, documentation issues, or performance concerns and escalate when needed, • Maintain accurate, audit-ready records and reporting, • Coordinate with statewide technical assistance providers to support regional coalitions, • Help regional partners meet CMS reporting, data collection, and outcome tracking requirements, • Serve as the day-to-day liaison between IDOH and regional stakeholders, including hospitals, clinics, EMS providers, and community organizations, • Support regional partners in understanding program requirements, solving problems, and keeping projects on track, • Bachelor’s degree in public health, health administration, public policy, business administration, or related field, • 5+ years of experience in grant management, contract oversight, program administration, or related work, • 3+ years of experience working with healthcare organizations, rural communities, or public health systems, • 2+ years demonstrating strong understanding of compliance, budgeting, and reporting requirements for state or federal grants, • Ability to manage multiple projects simultaneously with strong attention to detail, • Excellent communication, facilitation, and relationship-building skills, • Ability to work independently in a remote, field-based role, • Experience with CMS-funded programs or federal cooperative agreements, • Familiarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations, • Experience collaborating with statewide associations or multi-stakeholder coalitions *Regions - Must reside in one of these locations • Region #1- Includes: Elkhart, Fulton, Jasper, Marshall, Newton, Pulaski, and Starke, • Region #2 - Includes: LaGrange, Steuben, Dekalb, Noble, Kosciusko, Cass, Miami, Wabash, Huntington, Whitley, Wells, Adams, • Region #3 - Includes: Benton, White, Carroll, Clinton, Warren, Fountain, Montgomery, Boone, • Region #4 - Includes: Grant, Blackford, Jay, Tipton, Madison, Randolph, Henry, Wayne, Fayette, Union, • Region #5 - Includes: Vermillion, Parke, Putnam, Clay, Owen, Sullivan, Greene, • Region #6 - Includes: Shelby, Rush, Decatur, Franklin, Jennings, Ripley, Dearborn, Scott, Jefferson, Switzerland, Ohio, • Region #7 - Includes: Brown, Lawrence, Jackson, Orange, Washington, Crawford, Perry, Harrison, • Region #8 - Includes: Knox, Daviess, Martin, Gibson, Pike, Dubois, Posey, Vanderburgh, Warrick, Spencer
