Working collaboratively with GPs, Advanced Clinical Practitioners, pharmacists, nursing staff and the wider multidisciplinary team, you will help deliver safe, effective and accessible care to our patient population.The role will involve managing a varied caseload, assessing and treating patients, supporting long-term condition management, and contributing to proactive and preventative healthcare.
