Job Description Education and / or Experience: • Minimum of 2 years of full-time work experience, preferably in healthcare, or equivalent academic background., • High school diploma or equivalent required., • Knowledge of healthcare and managed care practices is preferred., • Typing speed of at least 60 words per minute with high accuracy., • Proficient in the use of computers, printers, faxes, and other office equipment., • Skilled in MS Office Suite (Word, Excel, Outlook, Access, PowerPoint)., • Demonstrated ability to handle confidential information with discretion., • Strong multitasking skills with the ability to manage multiple responsibilities simultaneously., • Familiarity with medical terminology., • Excellent phone etiquette and communication skills. The Post Discharge Care (PDC) Coordinator Lead plays a pivotal role in overseeing and optimizing the discharge planning process, ensuring the seamless creation and follow-up of post-discharge appointments. In this position, the PDC Coordinator is responsible for the daily monitoring, updating, and maintenance of follow-up grids across all campuses. The primary goal is to provide critical support to the inpatient management process by ensuring that accurate and timely information is communicated to the RMG administrative team, hospitalists, care managers, and outpatient teams. Essential Duties and Responsibilities include the following: • Oversee and support team activities, including monitoring performance, scheduling, documentation, audits, evaluations, and assisting program managers during staffing shortages., • Update and maintain key department documentation such as Data Collection Logs, Time Logs, and other materials as needed., • Maintain organization, meet departmental goals, and ensure the timely submission of high-quality deliverables to the San Fernando/Simi Valley medical management team., • Work cooperatively with the executive team, Medical Directors, and other stakeholders to support team initiatives., • Identify and recommend new approaches to enhance efficiency, production, and quality within the department., • Anticipate and address customer needs by listening for cues and responding appropriately with the necessary level of intervention., • Assist and support colleagues, fostering a collaborative team environment., • Communicate clearly and effectively with management, coworkers, and physicians, ensuring timely follow-through on requests and information sharing., • Demonstrate honesty and integrity in all activities, acknowledging errors and reporting them to the appropriate manager immediately., • Protect patient and employee privacy by ensuring that all personal health information remains confidential., • Serve as a liaison between care managers, patients, physicians, and other healthcare providers to ensure comprehensive care., • Contribute to the efficient use of resources, including supplies and equipment, to promote cost-effective practices., • Assist Inpatient Care Managers with discharge planning by generating authorizations and scheduling necessary services within department timelines., • Support the safe transition of patients from inpatient to home care settings by ensuring all medically necessary services are delivered., • Identify network needs and report potential contracting opportunities to management., • Review case statuses to ensure proper documentation and timely case closure in collaboration with assigned Care Managers.
