Position Code: OOTC Exempt Status/Pay Type: Non-Exempt Reports to: 0 DOL Status: Full Time Department: PTC Work Schedule: Typically, M-F 8am to 5pm Location: Higgins WC Code: 8868 Travel: 1%-10% EEO-1 Category: SW GENERAL DESCRIPTION OF POSITION This position is responsible for providing direct therapeutic services to children for acquisition and mastery of functional skills in order to reduce or overcome limitations resulting from disabilities or developmental delays. This position carries out treatment plans, documents progress and required processes, educates families and community partners and collaborates with physicians and other care providers. Services are clinic-based in both individual and group settings. ESSENTIAL FUNCTIONS This position requires face to face interactions with staff, board, vendors, and the general public on a daily basis to accomplish all essential functions of the position. • Participates in monthly team meetings, discipline, agency, and staffing meetings to ensure family and child outcomes are met and required information is secured. These indicators provide quantifiable measures that reflect how effectively an employee is performing their duties and contributing to the overall goals and objectives of the organization. In a performance review, performance indicators serve as benchmarks against which an employee's performance is assessed. Performance indicators should be specific, measurable, achievable, relevant, and time-bound (SMART) to ensure clarity and objectivity in the review process. Direct Service: Ensure services are being provided as stated on the POC. OT will provide a minimum of the daily impact rate (DIR) on all days worked. This includes direct service of treatment. Expectation will be measured on the Average Visits per Day Summary report. Expectation: 6-6.6 Daily Impact Rate 25% Parent Satisfaction Ensure that services and effective coaching is provided to families. This performance indicator will be reported from the "Satisfaction Survey". The survey will be sent to parents on a semester basis. Expectation score of 3-4.74. 25% Timely and Accurate Documentation Ensure that all paperwork deadlines are met so services can be billed in a timely manner. Daily notes are due by the following business day by 11:59 pm. All evaluation notes must be submitted by five business days of evaluation completion. Expectation will be measured on the Paperwork %. Expectation: 90-100% 25% Service Continuity Ensure there are no gaps in service by scheduling any re-evaluations in a timely manner with the supervising OT (at least one month prior to expiration). This will be measured through myEvolv report. Expectation: 95-100% 25% I have received and understand the performance indicators outlined for my performance review.
