Job DescriptionDescription: BASIC FUNCTION: Position is responsible for providing comprehensive nursing services to clients and their families including assessment and education in coordination with team-based care. Performs care management of patients with complex conditions within the primary care and family practice setting. SKILLS AND ABILITIES: • Excellent clinical skills., • Excellent telephone skills., • Excellent customer service and teamwork skills., • Graduate of an accredited school of nursing required; BSN preferred., • Licensure as a Registered Nurse in the State of Missouri required, and in good standing.
