Position Summary: The Clinical Education Manager is a strategic clinical leadership role responsible for elevating DHM Agency's clinical excellence through innovative education programs, hospital partnership development, and quality improvement initiatives. This position combines direct patient care with program development to support our educational endeavors. Personality Characteristics • Critical Thinker, • Strong multitasker, • Excellent communicator, • Detail-oriented, • Proactive problem-solverCORE RESPONSIBILITIESClinical Program Development, • Design and implement comprehensive orientation programs for new nursing staff, • Develop clinical pathways for high-acuity home health care, • Create and maintain clinical education curricula for ongoing staff development, • Establish programs and frameworks, • Lead quality improvement initiatives and clinical outcome analysis, • Develop clinical protocols for complex cases (tracheostomy care, IV therapy, wound management)Partnership & Education, • Serve as clinical liaison to hospital systems throughout Central Ohio, • Develop and deliver educational presentations to hospital discharge planning teams, • Create and facilitate "Learning" sessions on home health, • Build collaborative relationships with hospital case managers and clinical staff, • Provide consultation on complex discharge planning cases, • Represent DHM Agency at healthcare professional meetings and conferencesDirect Patient Care & Case Management, • Provide skilled nursing visits to approximately 20 patients per month, • Conduct comprehensive assessments and develop individualized care plans, • Manage complex, high-acuity cases requiring specialized nursing skills, • Coordinate interdisciplinary care with physicians, therapists, and other healthcare providers, • Complete OASIS assessments, care plan updates, and clinical documentation, • Serve as clinical resource for challenging patient situationsStrategic Leadership, • Participate in organizational strategic planning and clinical decision-making, • Analyze market trends and competitive landscape in home health services, • Develop metrics and reporting systems for clinical quality and partnership success, • Collaborate with leadership on business development through clinical excellenceQUALIFICATIONSRequired:, • Master's degree in Nursing (MSN) preferred;
